In accordance with an oral request received September 17, 2015 from Wyatt Edsel, we are
forwarding herewith a correction in Page 1 of Permit Cover Letter for Permit No. WQ0004797 issued
September 3, 2015.
The correction to the subject permit cover letter is as follows:
"In the list of modifications, point 1: 'Increase the non -discharge permitted flow from
83, 000 to 99,900 gallons per day (GPD), and the wastewater treatment plant capacity to 185, 000
GPD, rr
should be changed to:
"Increase the non -discharge pertnitted flow from 83,000 to 99,900 gallons per day (GPD), and the
wastewater treatment plant capacity to 164,900 GPD."
Note that the value in the permit is correct.

Dear Mr. Kehoe:
In accordance with your permit renewal with major modification request received June 8, 2015,
and subsequent additional information received July 28, 2015, and August 31, 2015, we are forwarding
herewith Permit No. WQ0004797 dated September 3, 2015, to Clement Pappas NC, LLC for the
construction and operation of the modified facilities, as well as the continued operation of the existing
wastewater treatment and irrigation facilities.
The modifications to the subject permit are as follows:
1. Increase the non -discharge permitted flow from 83,000 to 99,900 gallons per day (GPD), and
the wastewater treatment plant capacity to 169,900 GPD.
2. Removal of a 1.75 -MG Iagoon from the permit since it was not constructed.
3. Addition of four new aerators.
This permit shall be effective from the date of issuance until August 31, 2020, shall void Permit
No. WQ0004797 issued October 12, 2012 and corrections made November 19, 2012, and shall be subject
to the conditions and limitations as specified therein. Please pay particular attention to the monitoring
requirements listed in Attachments A, B and C for they may differ from the previous permit issuance.
Failure to establish an adequate system for collecting and maintaining the required operational
information shall result in future compliance problems.

Please nate the following permit conditions have been removed ,since the last permit
issuance dated October 12, 2012:

Please note the following permit conditions are new since the last permit issuance dated
October 12, 2012:
1. Condition I.3.: A copy of the permit with the City of Hendersonville for discharge to the
City's wastewater treatment plant to be provided to the Asheville Regional Office.
Although the wastewater treatment plant flow has increased from 83,000 to 164,900 gallons per
day (GPD), the permitted non -discharge flow to the irrigation fields is limited to 99,900 GPD, with the
balance going to the dissolved air flotation (DAF) unit and the City of Hendersonville's wastewater
treatment plant. As a result the monitoring requirements of both effluent wastewater stream to the spray
field and the groundwater monitoring requirements remain unchanged.
If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee
has the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be final and
binding.
