Division of Water Resources' Central and Regional staff has reviewed the application package received
June 2, 2015. However, additional information is required before the review may be completed. Please
address the items on the attached pages no later than the close of business on September 14, 2015.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and
regulations. Any oversights that occurred in the review of the subject application package are still the
Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in
Sections A through B, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All revised
and/or additional documentation shall be signed, sealed and dated (where needed), with three copies
submitted to my attention at the address below.

A. Water Balance:
Is there 83,000- (telephone conversations), 120,000- (water balance calculations provided) or
185,000 -gallons (cover letter) per day going to the irrigation fields? This will be the amount
permitted and will also determine the frequency of monitoring for both the effluent and the
groundwater. The threshold for going to once per month for effluent and 4/year for groundwater
is 100,000 -gallons per day flow. In any case, please submit signed and sealed water balance
calculations as required under 15A NCAC 02T .1104(a)(2)(C). This will be the amount
permitted. The 65,000 -gallons going to the DAF unit is immaterial to the permit.
B. EnLyineering Plans and Specifications:
The aerators are in fact to become part of the permit. Submit signed and sealed drawings and
specifications of the aerators as required under 15A NCAC 02T .1104(a)(2)(A) and (B). These
drawings and specifications must be signed either by an Aqua Jet Professional Engineer if their
documentation is enclosed or by an Altamont Environmental Professional Engineer if these
drawings and specifications are incorporated into Altamont's documents as required by the North
Carolina Board of Engineers and Land Surveyors (NCBELS).
