permit renewal application. Specifically, these fields are: 1, 2, 3, 4, 5A, 5B, 6, 7A, 7B, 8, 9, 10
and 11. Field 5 is shown as one field on the map submitted, but fields 5A and 5B are listed on the
submission. Please show how the field is subdivided and setbacks. In addition, please show
fields 6 through 11 on a map.
2. The monitoring wells shown on the site map submitted had MW -07 at the location of MW -07R_
In addition, inactive wells MW -01, MW -02, MW -02R, and MW -03 are shown when they should
not be.
