Dear Permittee:
The Division of Environmental Management issued the subject permit on
3/07/91 for the construction and/or operation of a subsurface wastewater
treatment and disposal system. This Division processed a nondischarge
permit under the jurisdiction of North Carolina General Statutes
143-215.1 and 130A-335 and North Carolina Administrative Code Section 15
NCAC 2H .0200; "Waste Not Discharged to Surface Waters" which was issued
until rescinded.
On July 14, 1992, House Bill 1545 was ratified in the North Carolina
General Assembly which transferred the authority for the plan review,
permit issuance and compliance monitoring of most Environmental
Management Commission subsurface wastewater treatment and disposal
systems, whose Staff body is the Division of Environmental Management, to
the COmm i4EA.,Ci for Health Services through the local health departments
and the Divisiozx of. Environmental Health. Under this new authority, the
nondischarge permit for the wastewater treatment and disposal system
under Permit No WQ0004746 is now under the ,jurisdiction of the HARNETT
County Health Department and the Division of Environmental Health. Under
this new authority, any renewal requests, or requests for modifications,
upgrades or repairs of this system will now be reviewed and processed by
the HARNETT County Health Department.

Dear Mr. Wortman:
In accordance with the permit application received from the Barnett
County Board of Education on February 21, 1991, and supporting information
provided by the engineering firm of McKim and Creed, we are forwarding
herewith Permit No. WQ0004746 to the Harnett County Board of Education. This
permit is for the continued operation of an existing septic tank nitrification
field system serving Lafayette School in the Kipling community of Harnett
County.
This permit shall be effective from the date of issuance until rescinded
and shall be subject to the conditions and limitations therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request
must be in the form of a written petition, conforming to Chapter 150E
Administrative Hearing, P.O. Drawer 11666, Raleigh, NC 27604. Unless such
demands are made, this permit shall be final and binding.

In accordance with the provision of Article 21 of Chapter 143, General
Statutes of forth Carolina as amended and other applicable Laws, Rules, and
Regulations,
PERMISSION IS HEREBY GRANTED TO

continued operation of two (2) separate septic tank -nitrification field
systems, one consisting of a 1,600 -gallon grease trap, a 840 -gallon septic
tank, distribution box, and a total of 1,800 sq. ft. of nitrification field in
two (2) foot trenches. The second system consists of a 7,400 --gallon septic
tank, distribution box, and a total of 11,160 sq. ft. of nitrification trench
in 6 -ft. trenches, pursuant to the application received February 21, 1991, and
in conformity with the project plans and other supporting information
subsequently filed, and approved by the Department of Environment, Health, and
Natural Resources and considered a part of this Permit.
This Permit shall be effective from the date of issuance until rescinded
and shall be subject to the following specified conditions and limitations:
1. This Permit shall become voidable unless the facility is
operated in accordance with the approved plans and other
supporting data.
2. This Permit is effective only with respect to the nature and
volume of wastes described in the application and other
supporting data.
3. The facility shall be properly maintained and operated at all
times.
4. This Permit is not transferable.

17. The nitrification field shall be properly graded and seeded to
ensure prompt runoff of surface water.
