Dear Mr. Miller:
The Division of Environmental Management has received information from the Winston Salem
Regional Office which confirms that petroleum contaminated soils frorn Sequoia Supply have been
remediated. Therefore, as you requested, Permit Number WQ0004628 is hereby rescinded effective
immediately.
1f in the future, you detem-Ane that you want to treat petroleum contaminated soils, you must first
apply for and receive a new Nondischarge permit. Failure to obtain a valid Nondischarge Permit for such
activities will subject the owner to a civil penalty of up to $10,000 per day.
