Dear Mr. Jennings:
In accordance with your application received November 8, 1990, and
supplemental information you provided on December 2, 1991, we are
forwarding herewith Permit Number WQ0004:378 dated December 9, 1991, for
the construction and operation of a septic tank, low-pressure pipe
subsurface wastewater disposal system to serve fish process and
domestic wastewater from Jennings Trout >aat:m.
This Permit will be effective from t=lre date of its issuance until
December 1, 1996, and shall be subject to the conditions and
limitations as specified therein.
If any parts, requirements, or limitations contained in this
permit are unacceptable to you, you have the right to request an
adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request must be in the form of a written
petition, conforming to Chapter 150B of Nor.tlh Carolina General Statutes
and filed with the Office of Administrative Hearings, P.O. Box 11666,
Raleigh, NC 27604. Unless such demands are made this permit shall be
final and binding.

In accordance with the provisions df Article 21 of Chapter 143,
General Statutes of North Carolina as amenried, and other applicable
Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO
JENNINGS TROUT FARM
FOR THE
construction and operation of a 950 GPD sul�surface wastewater treatment
ind disposal system consisting of a 2000 gallon septic tank, a 2000
jallon pump tank with dual 30 GPM pumps with timer control and high
water alarm, approximately 400 L.F. of 2 -inch PVC forcemain, 40 L.F. of
1.5 -inch PVC manifold and seven (7) distribution laterals totaling 355
L.F. of trench to serve fish process washdown and domestic wastewater
from Jennings Trout Farm in Haywood County with no discharge to surface
waters, pursuant to the application received on November 8, 1990, and
in conformity with the project plans, specifications, and other
suppoFting data, subsequently filed with fAie Department of Environment,
Health, and Natural Resources and consider -d a part of this Permit.
This Permit shall be effective from the date of its issuance until
December 1, 1996, and shall be subject to the following specified
conditions and limitations:
1. Flow limiting devices shall_ be ttseci on all facilities
at the Jennings Trout Farm
2. This Permit is effective only wi_1-1h respect to the
nature and volume of wastes described in the
application and other supporting (73ata.
3. This Permit shall become voi.dabl�- in event of failure
of the soil to adequately absorb the wastes, and may
be rescinded unless the facilities are installed,
maintained, and operated in a manner which will protect
the assigned water quality standards of the surface
and ground waters.
4. This facility shall not dischar,cl- directly to the surface
waters of the State.

5. In the event the facilities fail t:o perform satis-
factorily, the owners shall take such immediate
corrective action as may be required by this Depart-
ment.
6. This Permit is not transferable.
7. The method for final disposal',of screenings, scum,
and sludges from these facil�ies shall not result
in any contamination of the surface or ground waters
of this State_
B. This disposal system shall be abandoned immediately
upon the availability Of a regional sewer system.
9. Any monitoring deemed necessary by the Division of
Environmental Management to insure the protection of
ground waters will be established and an acceptable
sampling and reporting program shall be followed.
10. The approved nitrification field site must not be
disturbed, regraded, or substi.tLited without Written
permission from the Asheville Regional Supervisor.
