This permit shall be effective from the date of issuance until rescinded and shall be subject to the conditions and limitations therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearing, P.O. Drawer 11666, Raleigh, NC 27604, Unless such demands are made, this permit shall be final and binding. MJN/KA/rhg cc: Moore County Health Department Fayetteville Regional Supervisor Hobbs, Upchurch and Associates Groundwater Section Dennis Ramsey K m 'Si rel .�. �7?�:s��r,SL1p�X':V�i:S4t?.•' ..r�.•;`{��.3sd NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT, HEALTH AND NATURAL RESOURCES RALEIGH PERMIT For the Discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provision of Article 21 of Chapter 143, General Statutes of North Carolina as amended and other applicable Laws, Rules, and Regulations, PERMISSION IS HERESY GRANTED TO Moore County Emergency Management Union Pines Advanced Life Support Facility Moore County FOR THE construction and operation of a septic tank -nitrification field system consisting of a 900 gallon septic tank, a distribution box, and a total of 160 L.F. of nitrification line installed in 3 -foot trenches, pursuant to the application received October 15, 3990, and in conformity with the project plans, speci£ications,and other supporting data, subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a part of this Permit. This Permit shall be effective from the date of issuance until rescinded and shall be subject to the following specified conditions and limitations: 1. This Permit shall become voidable unless the facility is constructed in accordance with the approved plans, speci- fications, and other supporting data. 2. This Permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facility ,shall be properly maintained and operated at all times. 4. This Permit is not transferable. Moore County Emergency Management Union Pines Advanced Life Support Facility Page 2 5. Solids, sludges, or other pollutants removed or result- ing from the wastewater storage facility shall be contained and disposed of in such a manner as to prevent any contamination of the surface or groundwater of the State. 6. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all times. 7. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facility. 8. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal main- tenance is being performed. 9. The facility shall be effectively maintained and operated as a nondischarge system to prevent the discharge of any wastewater resulting from the operation of this facility. 10. The issuance of this Permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 11. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 12. Monitoring wells shall be constructed and sampling schedules arranged as required by the Division of Environmental Management and in accordance with 15 NCAC 2C .0100. 13. Any monitoring deemed necessary by the Division of Environ- mental Management to insure surface and groundwater protection will be established and an acceptable sampling reporting schedule shall be followed. 14. The Fayetteville Regional Office, phone number (919) 486-15411 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed subsurface disposal system so that an in-place inspection can be made of said system prior to backfilling. Such notification to the Regional Supervisor shall be made during the normal office hours from 8 a.m. until 5 p.m. Monday through Friday, excluding state holidays. In event the facility fails to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or a dis- posal facility. 16. This Permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facility is installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and groundwaters. 17. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. 18. All surface drainage on this site shall be diverted away from any portion of the nitrification field or designated repair area. 19. The nitrification field shall be properly graded and seeded to ensure prompt runoff of surface water. 20. The invert of the nitrification lines shall not exceed a depth of 36 inches below natural ground level.
