Dear Mr. Balser:
In accordance with a telephone request from your staff to Ms. Tara MacPherson of the Division
of Water Quality's (Division) Wilmington Regional Office that was subsequently forwarded to the
Division's Central Office on October 13, 2004, we are forwarding herewith a corrected Page 5 of Permit
No. WQ0004075, which was issued on September 23, 2002, for the subject surface irrigation facilities.
The following correction has been made to the permit:
Condition IV. 1. has been corrected to reflect the fact that an analysis for volatile organic
compounds (VOCs) present in the groundwater monitoring wells should be completed during
the October sampling event (i.e., a separate sampling event in September is not needed). The
original listing of September as the analytical month for VOCs was a typographical error that
was created when the Division prepared the September 23, 2002 issuance of the permit. The
Division apologizes for any confusion that this error might have caused.
Please replace Page 5 with this corrected Page 5 into Permit No. WQ0004075, issued on
September 23, 2002.
Ms. MacPherson also related that your staff would like to explore modifying further the months
during which sampling of the wastewater effluent and groundwater monitoring wells must occur. To
request a change in these monitoring months, you may submit a minor permit modification request to the
Division for review and approval. This request should be in narrative form, providing detail as to what
modification is desired as well as a justification for the request, and submitted to the Land Application
Unit at the address at the bottom of this page. No application fee is required to process minor
modification requests.

b. Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate wastewater treatment such as mechanical or electrical failures of
pumps, aerators, compressors, etc.
Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass
directly to receiving waters without treatment of all or any portion of the influent to such
station or facility.
d. Any time that self -monitoring information indicates that the facility has gone out of
compliance with its permit limitations.
Persons reporting such occurrences by telephone shall also file a written report in letter form
within five (5) days following first knowledge of the occurrence. This report must outline the
actions taken or proposed to be taken to ensure that the problem does not recur.
IV. GROUNDWATER REQUIREMENTS
Monitor wells MW-1, MW-2, and MW-4, shall be sampled EVERY APRIL, AUGUST and
OCTOBER, for the parameters listed below. Prior to sampling the parameters listed below, the
measurement of water levels must be taken. The depth to water in each well shall be measured from
the surveyed point on the top of the casing. The measuring points (top of well casing) of all
monitoring wells shall be surveyed relative to a common datum.
Water Level Total Ammonia Nitrogen Nitrate (NO3)
Total Dissolved Solids (TDS) Total Organic Carbon (TOC) Copper
Chloride Total Phosphorus Fecal Coliform
Volatile Organic Compounds (Sampled in October only, Standard Method 623OD, (PQL) at:
0.5 ug/L or less)
If TOC concentrations greater than 10 mg/L are detected in any downgradient monitoring well,
additional sampling and analysis must be conducted to identify the individual constituents comprising
this TOC concentration. If the TOC concentration as measured in the background monitor well
exceeds 10 mg/L, this concentration will be taken to represent the naturally occurring TOC
concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient
wells shall be subject to the additional sampling and analysis as described above.
If any volatile organic compounds (Sampled in September only) are detected by the 6230D method
(Capillary -Column, Standard Methods for the Examination of Water and Wastewater. 17'l' edition,
1989.), then the Wilmington Regional Office Groundwater Supervisor, telephone number (910) 395-
3900, must be contacted immediately for further instructions regarding any additional follow-up
analyses required. The results of all initial and follow-up analyses must be submitted simultaneously.
The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality
Monitoring: Compliance Report Form) by the Groundwater Section, Permits and Compliance Unit,
1636 Mail Service Center Raleigh, N.C. 27699-1636 on or before the last worldng day of the month
following the sampling month. The most recent form can be downloaded from
http://gw.ehnr.state.nc.us/ or requested from the address above. Laboratory analysis results must be
attached with the form.
5. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be
provided.
