Please summarize any findings resulting from this review: A review of self-monitoring data indicates one NDMR fecal
coliform violation (monthly geometric mean) in November 2016. Monitoring well #1 appears to have violations for total
dissolved solids and Nitrate Total Nitrogen with each GW -59 turned in in the last 12 months. There have also been a few
GW -59 monitoring frequency violations in the last 12 months. Sugarloaf Utility was assessed a civil penalty in March 2017
for failure to apply for a permit renewal.
Provide input to help the permit writer evaluate any requests for reduced monitoring, if applicable.
