Dear Mr. Ricks:
This letter is in reference to the Confirmation of Permit Revocation letter dated
December 15, 1993. You were informed that your permit had been revoked effective
November 29, 1993 for non-payment of your 1993 Annual Administering and Compliance
Monitoring Fee. The information submitted by Mr. Cook showed that the fee had been paid
on July 29, 1993. Our review indicates that the fee was paid and the check was cashed by
the Division of Solid Waste Management. The Confirmation of Permit Revocation letter was
sent to you in error. State Permit No. WQ0003522 has been reinstated.
This permit expires on May 31, 1995. Please be advised that the permittee, at least 180
days (6 months) prior to the expiration of this permit, must request its renewal.
We regret any inconvenience this error may have caused and are taking measures to
reduce these kinds of errors in the future.
