In accordance with an e-mail request received on June 11, 2004, from Ms. Mary Buck of Synagro
South, Inc., we are forwarding herewith a corrected Page 1 and Page 2 of Attachment B, certified on June
2, 2004, of Permit No. WQ0003504, which was issued on June 18, 2002, for the subject residuals land
application program.
The following corrections have been made to Attachment B:
1. During the site visit that was performed by the Division of Water Quality's (Division)
Raleigh Regional Office, it was discovered that one land application site that was listed in the
permit modification application package as being in Orange County was, in fact, located in
Caswell County. The Attachment B was written accordingly; however, the original land
application site identification number (i.e., Site No, NC-OR-92-02) was retained. Ms. Buck
has requested that this number be changed to Site No. NC-CA-92-02 to correspond with
Synagro South Ine.'s standard land application site identification numbering system.
2. The last name of the operator/lessee for Site No. NC-OR-59-02 was accidentally spelled
incorrectly in the Attachment B, and the error has been corrected (i.e., the correct spelling is
Hawkins).
3. The last name of the landowner for Site No. NC-OR-72-01 was accidentally spelled
incorrectly in the Attachment B, and the error has been corrected (i.e., the correct spelling is
Crabtree).
The Division apologizes for any confusion that these errors might have caused, and we
appreciate Ms. Buck's diligence in reviewing this permit thoroughly and calling these issues to our
attention.
Please replace Page 1 and Page 2 of Attachment B, certified on June 2, 2004, with this
corrected Page 1 and 2 of Attachment B, certified on June 2, 2004, into Permit No. WQ0003504,
issued on June 18, 2002.
