In accordance with your permit renewal request received January 7, 2004, we are forwarding
herewith Permit No. WQ0003301, dated January 24, 2005, to Willie Bethune for the continued operation
of the subject single family wastewater treatment and surface irrigation facilities.
This permit shall be effective from the date of issuance until December 31, 2009, shall void
Permit No. WQ0003301 dated July 1, 1999 and shall be subject to the conditions and limitations as
specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to
establish an adequate system for collecting and maintaining the required operational information will
result ir future compliance problems.
During the site visit several items were noted that need attention. During the visit it was
indicated that the septic tank was pumped annually. This is probably more frequent than is
needed. The septic tank does not need to be pumped until there are eight to 12 inches of solids in
the bottom. The vehicles parked on the sand filters should be removed. There is a potential that
petroleum products can leak into the filters. The conduit through which the electrical cable that
leads from the pump to the control box should be sealed so corrosive gases do not enter the control
box. The fence around the spray field needs to be repaired.
At least six (6) months prior to the expiration of this permit, an extension shall be requested.
Upon receipt of the request, the Commission will review the adequacy of the facility described therein,
and if warranted, will extend the permit for such period of time and under such conditions and limitations,
as it may deem appropriate.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 15013 of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, NC 27699-6714. Unless such demands are made this permit shall be final and binding.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of Nojih Carolina as
amended, and other applicable Laws, Rules, and Regulations

continued operation of a 600 gallon per day (GPD) surface irrigation wastewater treatment and disposal
system consisting of a 1,200 -gallon baffled septic tank, a 300-A' subsurface sand filter (two fltcrs, size
unknown), a distribution box, a tablet chlorinator with a 75 -gallon chlorine contact chamber, a 2,400 -
gallon storage/pump tank with a single 23 -gallon per minute (GPM) irrigation punip and a 15,000-ft2
spray irrigation area served by six full -circle sprinklors and all associated piping and appurtenances, to
serve ihz Bethune residence with no discharge of wastes to the surface waters, pursuant to the application
received January 7, 2004, and in conformity with the project plan, specifications, and other supporting
data subsequently filed and approved by the Department of Environment and Natural Resources and
considered a part of this permit.
This permit shall be efi'ective from the date of issuance until December 31, 2009, shall void.
Permit No. WQ0003301 issued July 1, 1999 and shall bo subject to the following specified conditions and
limitations:
L PERFORMANCE STANDARDS
1, The surface in igation facilities shall bceffectively maintained and operated at all times so that
there is no discharge to surface waters, -nor-any contamination of ground waters, which will
render there unsatisfactory for normal use. In the event that the facilities fail to perform
satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to
adequately assimilate the wastewater, the, Permittee shall take immediate corrective actions
including those actions that may be required by the Division of Water Quality, such as the
construction of additional or replacement wastewater treatment and disposal facilities.
2. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to
surface or ground waters resulting from the operation of this facility.
3. The residuals generated from these treatment facilities must be disposed in accordance with
General Statute 143-215.1 and in a manner approved by the Division,
4. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited.

5. The following buffers shall be maintained;
a. 400 feet between wetted area and any residenoe or places of public assumbly under separate
ownership,
b. 150 feet between wott:ed Areaand property lues,
c. 100 feet between wetted area and wells,
d. 100 feet between wetted area and drainage ways or surface water bodies,
e. 50 feet between wetted area and public right of ways,
f' 100 feet between treatnient/storage units and any wells, and
g. 50 feet between treatment units and property lines.
Some of the buffers specified above tray not have been included in previous permits for this
waste treatment and disposal system. These buffers are not intended to prohibit or prevent
modifications, which are required by the Division, to improve performance of the; existing
treatment facility, These buffers do, however, apply to modifications of the treatment and
disposal facilities that are for the purpose of increasing the flow that is tributary to the facility.
Thesc buffers do apply to any expansion or modification of the Surface irrigation areas and
apply in instances in which the sale of property mould cause any of the buffets now complied
with, for the treatment and disposal facilities, to be violated. The applicant is advised that any
modifications to the existing facilities will require a permit modification.
6. The pump tank(s) shall have functional audible and visual high water alarms that are external to
any structure.
IL OPERATION ANQ MAINTTEnNLE REQIWZMFNTS
The facilities shall be properly maintained and operated at all times
2. Upon classification of the wastewater treatment and surface irrigation facilities by the Water
Pollution Control 5ystern Operators Certification Commission (WPCSOCC), the Permittee
shall designate and einploy a certified operator to be in responsible charge (ORC) and one or
more certified operator(s) to be hack -up ORC(s) of the facilities in accordance with 15A NC;AC
8G .0201. The ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as
STecified in this permit anal shall comply with all other conditions specified in these rules,
3. A suitable year round vegetative cover shall be maintained
4. Irrigation shall not be performed during inclement weather or when the ground is in a condition
that will cauw runoff.
5. Adcquate measures shall be taken to prevent wastrw&ter runoff from the irrigation field.
6. The Application rate shall not exceed a cumulative loading of 36.4 inches over any twelve (12)
month period at an instantaneous application rate not to exceed 0.2 inches per hour per day on a
three day cycle.
7. The facilities shall be. effectively maintained and operated as a non -discharge system to prevent
the di &charge of any wastewater resulting from the operation of this facility.
8. No type of wastewater other than that from the Gates residence shall be sprayed onto the
irrigation area.
9. No trafG4 or equipment shall be allowed on the disposal area except while installation occurs or
while normal mainterianQ0 is being performod,

11. Wastewater treatment units open to tha atmosphere shall have restricted access and pump
control panels shall be locked.
12, The Permittee is responsible for the operation and maintenance of the entire treatment and
disposal system including, but not limited to, the following items:
a. The septic tank shall be checked mmually and pumped out as needed. The septic tank
effluent filter shall be checked and cleaned at the same frequency of the septic tank.
b. The tablet chlorinator shall be Checked every week. Tablets shall be added to provide
proper chlorination.
c. The storage, pump, and alarm systems shall be inspected monthly. The scam layer shall be
removed at the same interval as the septic tank is cleaned out.
d. The system shall be inspected monthly to make certain of the proper operation of the spray
head(s), that the vegetative growth allows a proper spray pattern, that the soil is
assimilating the disposed treated wastewater with no surface runoff, and that no
objectionable odors are being generated,
ill. MONILTORING AND REPORTING NT
1. Any monitoring deemed necessary by the Division to insure surface unci ground water
protection will be established and an acceptable sampling reporting schedule shall be followed.
2. The Permittee shall maintain records of all maintenance performed on the system and irrigation
area, as required in Condition H(12) for a minimum of five years. This information shall be
provided to the Division upon request.
3. Noncompliance Noliiication:
The: Permittee shall report by telephone to the Raleigh Regional Office, telephone numbcr (919)
571•-4700, as soon as possible, but in no oaac more than 24 hours or on the next working day
following the occurrence or first knowledge of the occurrence of any of the following:
a. Any process unit failure, duo to down or unloiown reasons, that renders the facility
incapable of adequate wastewater troaknent, such as mechanical or electrical failures of
pumps, spray heads, etc.; or
b. Any failure of a pumping station or treatment facility resulting in a by-pass directly to
receiving waters without treatment of all or any portion of the influent to such station or
facility.
Occurrences outside normal business hours may also be reported to the Division's ]emergency
Response personnel at telephone number (840) 858-0368 or (919) 733-3300. Persons reporting
such occurrences by telephone shall also file a written report in Ictterform within five (5) days
following first knowledge of the occurrence. This report must outline the actions taken or
proposed to be taken to ensure that the problem does not recur,

INSPECTIONS

1. Adequate inspection, maintenanco, and cleaning shall be provided by the Permittee to insure
proper operation of the subject facilities.
Any duly authorized officer, employee, or representative of the Division may, upon
presentation of credentials, enter and irnapmt any property, premises or place on or related to
the disposal site or facility at any reasonable time for the purpose of determining compliance
with this permit, may inspect or copy arty records that must be maintained under the terms and
conditions of this permit, and may obtain samples of groundwater, surface water, or leachate,
GENERAL CONDITIONS
This permit shall become void unless the facilities are constructed in accordance with the
conditions of this perinit, the approved plans and specifications, and other supporting data.
2. This permit is effective only with respect to tho nature and volume of wastes described in the
application and other supporting data.
3. This permit is not transferable. In the event there is a desire for the facilities to change
ownership, or there is a narne change of the Permittee, a formal penult request must be
submitted to the Division accompanied by documentation from the parties involved, and other
supporting materials as may be appropriate. The approval of this request will be considered on
its merits and may or may not be approved.
4. Upon the availability of a municipal or regional sewerage collection system, the subject
wastewater treatment facilities shall be abandoned and all wastewater discharged into the
municipal or regional sewerage system.
5. Failure to abide by the conditions and limitations contained in this permit may subject the
Perrnittee to an enforcement action by the Division in accordance with North Carolina General
Statute 143-215.6A to 143-215.6C.
6. The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances, which may be imposed by other govenunent agencies
(local, state, and federal), which have jurisdiction.
7. This permit may be revoked if the Parmittee Perils to ;,bide by the conditions of the "Operation
and Maintenance Agreement" previously signed by the Permittee.
S. The Pcnnittee shall retain a set of approved plans, specifications of the subject facility for the
life of the project.
9. The Pennittce shall pay the annual administering and compliance fee within 30 days of being
billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate
action to revoke this permit as specified by 15A NCAC 2H .0205 (c)(4).

10. The Permittee, at least six (6) months prior to the expiration of this pen -nit, shall request its
extension. Upon receipt of the request, the Commission will review the adequacy of the
facilities described therein, and if warranted, will extend the Permit for such period of time and
under such conditions and litnitations, aa it shay deem appropriate.
