It has come to the Division's attention that a previous letter of this type sent to you was
misdated. This letter supersedes the letter dated August 3, 1989. The following deals with the
subject matter.
Per telephone conversations between you and Mr. Chris Lochlear and Mr. Lindsay Mize, the
Division of Environmental Management is in receipt of information which confirms that the subject
facility no longer wishes to have a nondischarge permit for the operation of the subject subsurface
absorption field. Therefore, as you requested, Permit No. WQ0003273 (12312 renewal) and
Permit No. 12312 are hereby rescinded effective immediately. Should you wish at some future
time to use the subject facilities as wastewater treatment facilities, it will require the issuance of a
new nondischarge permit. Furthermore, the original application fee that accompanied your request
for renewal of Permit No. 12312 will be refunded under separate cover.
