In accordance with your application received February 16,
1990 and revised March 8, 1990, we are forwarding herewith Permit
No. WQ0003102, dated March 28, 1990, for the construction and
operation of the subject wastewater treatment and disposal
facility.
This permit shall be effective from the date of issuance
until March 31., 1995, and shall'be subject to the conditions and
limitations as specified therein. .
If any parts, requirements, or limitations contained in this
permit are unacceptable to you, you have the right to an
adjudicatory hearing before a hearing officer upon written
request within thirty (30) days following receipt of this permit.
This request must be in the form of a written petition,
conforming to Chapter 150E of the North Carolina General
Statutes, and filed with the Office of Administrative Hearings,
Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless
such demands are made, this permit shall be final and binding.

P E R M I T
For the Discharge of Sewage, Industrial Wastes or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143,
General Statutes of North Carolina as amended, and other applicable
Laws, Rules, and Regulations:
PERMISSION IS HEREBY GRANTED TO
Pender County Board of Education
for the
Continued operation of an existing septic tank and the construction
and operation of a 2400 GPD Low Pressure Pipe disposal facility
consisting of a dual pump dosing chamber, approximately 910 linear
feet of 3 inch force main, a LPP disposal field of 4000 sq. ft.,
piping, fittings and appurtenances to serve Topsail Elementary
School with no discharge to surface waters of the State.
This permit is granted pursuant to the application received
February 16, 1990 and revised March 8, 1990 in conformity with the
project.plans, specifications, and other supporting data,
subsequently filed and approved by -the Department of Environment,
Health and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until
March 31,1995 and shall be subject to the following specified
conditions and limitations:
1. This permit shall become voidable unless the facilities are
constructed in accordance with the approved plans,
specifications and other supporting data.

2. This permit is effective only with respect to the nature and
volume of wastes described in the application and other
supuortina data.
3. Adequate inspection, maintenance, and cleaning shall be
provided by the permittee to insure proper operation of the
subject facilities.
4. This permit is not transferable.
5. Upon the availability of a municipal or regional sewerage
collection system, the subject wastewater treatment system
shall be abandoned, and all wastewater discharged into the
municipal or regional sewerage system.
6. This permit shall become voidable in the event of failure of
the soil to adequately absorb the wastes, and may be rescinded
unless the facilities are installed, maintained and operated
in a manner which will protect the assigned water quality
standards of the surface waters and groundwaters.
7. The Wilmington Regional office, Phone No. 919/256-4161 shall
be notified at least twenty-four (24) hours in advance of
backfilling of the installed system so that an in-place
inspection can be made of said system prior to backfilling.
Such notification to the Regional Supervisor shall be made
during the normal office hours from 8:00 a.m. until 5:00 p.m.
on Monday through Friday, excluding State Holidays.
8. Any monitoring deemed necessary by the Division of
Environmental Management to insure surface and groundwater
protection will be established and an acceptable sampling
reporting schedule shall be followed.
9. The sludge generated from these treatment facilities must be
disposed of in accordance with the General Statutes 143-215.1
and in a manner approvable by the North Carolina Division of
Environmental Management.
10. The wastewater treatment system shall be effectively
maintained and operated at all times so that there is no
discharge to the surface waters, nor any contamination of
groundwaters which will render them unsatisfactory for normal
use. In event the facilities fail to perform satisfactorily,
including the creation of nuisance conditions, or failure of
the irrigation area to adequately absorb the wastewater, the
Permittee shall take such immediate corrective action as may
be required by the Division of Environmental Management,
including the construction of additional or replacement
wastewater treatment or disposal facilities.

11. Diversion or bypassing of the untreated wastewater from the
wastewater treatment system is prohibited.
12. The issuance of this permit shall not relieve the permittee of
.the responsibility for damages to surface or groundwaters
resulting from the operation of this facility.
13. No traffic or equipment shall be allowed on the disposal area
except while installation occurs or while normal maintenance
is being performed.
14. An acceptable reserve area as indicated on the approved plans
shall be maintained at all times.
15. The septic tank must be inspected at least twice annually and
pumped as needed to remove excessive floating and settled
solids.
16. The low pressure pipe disposal lines shall be purged at least
twice annually and the pressure adjusted to the approved
specifications.
17. A suitable vegetative cover shall be maintained on the low
pressure pipe field and the field shall be kept mowed. The
clippings shall. be removed to prevent the build of thatch.
