Paul Wilms Director SUBJECT: Amendment to Permit No. 6446 Summey Building Systems, Inc. Dunescape Villas Wastewater Treatment Plant Modifications Carteret County In accordance with a letter dated December 15, 1987, we are amending Permit No. 6446 to allow the construction and operation of an additional 65 feet diameter hydraulicaly driven rotary distributor with all the necessary piping, valves, appurtenances, a 200 GPM dosing pump and 247 linear feet of 4-inch effluent force main. This permit amendment shall be subject to the following additional specified conditions. ]1. Within 90 days of permit renewal, the two (2) existing monitor wells must be abandoned in conformance with the requirements of 15 NCAC 2C.0113 due to their improper construction. 12. Within 90 days of permit renewal, three (3) new monitor wells must be installed to monitor groundwater quality upgradient and downgradi-ent of the rotary distributors. The location and construction details for these wells must be by approval of the Wilmington- Regional Office, from which a well construction permit must be obtained. 13. The monitor wells must be sampled initially after construction and thereafter every March, July, and November for the following parameters: NO3 pH Total Ammonia TOC TDS Chloride Water Levels Orthophosphate Total Coliforms NO2 TOX (In November Only) 1 {• � LS's y'1 �- 1 Pallulian Precrniian Pars P.O. The analytical methods used for TOX must be capable of & all halogenated organic compounds present at a concentra billion (ppb) or greater. In the vent that a TDX concen greater is detected, any individual halogenated organic a concentration at or above the method detection limit (I identified and quantified utilizing EPA methods 601, 602 The results of the sampling and analysis shall be sent t+ Division of Environmental Management on form GW-59 (Comp Form) every April, August and December. 1.4. Any additional groundwater quality monitoring, as deemed Department, shall be provided. 15. The surface elevation shall be 7.5 feet MSL or greater. 16. The rotary distributors shall be maintained by raking or as necessary to keep the surface free of vegetation. 17. Fill material shall be approved by the Wilmington Region4 submittal of sieve analysis test results. 18. Upon completion of construction and prior to operation of facility, a certification must be received from a proles: certifying that the permitted facility has been installec approved plans and specifications.
