WQ0003030 Subsurface Wastewater Treatment & Disposal System PENSKE TRUCK LEASING -TRUCK WA COLUMBUS COUNTY Dear Permittee: The Division of Environmental Management issued the subject permit on 3/02/90 for the contruction and/or operation of a subsurface wastewater treatment and disposal system. This Division processed a nondischarge permit under the jurisdiction of North Carolina General Statutes 143-215.1 and 130A-335 and North Carolina Administrative Code Section 15 NCAC 2H .0200; "Waste Not Discharged to Surface Waters" which was issued until 3/31/95. On July 14, 1992, House Bill 1545 was ratified in the North Carolina General Assembly which transferred the authority for the plan review, permit issuance and compliance monitoring of most Environmental Management Commission subsurface wastewater treatment and disposal systems, whose staff body is the Division of Environmental Management, to the Commission for Health Services through the local health departments and the Division of Environmental Health. Under this new authority, the nondischarge permit for the wastewater treatment and disposal system under Permit No WQ0003030 is now under the jurisdiction of the COLUMBUS County Health Department and the Division of Environmental Health. Under this new authority, any renewal requests, or requests for modifications, upgrades or repairs of this system will now be reviewed and processed by the COLUMBUS County Health Department. At least 180 days prior to the expiration date of this permit of 3/31/95, you must contact the COLUMBUS health department and apply for an operation permit for the continued operation and maintenance of this system. This permit shall be effective from the date of issuance until March 31, 1995, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you,• you have the right to an adjudicatory hearing before a hearing officer upon written request within thirty (30) days following receipt of this permit. NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION RALEIGH DEPARTMENT OF ENVIRONMENT, HEALTH, AND NATURAL RESOURCES P E R M I T For the Discharge of Sewage, Industrial Wastes or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations: PERMISSION IS HEREBY GRANTED TO Penske Truck Leasing Company for the Construction and operation of a 500 GPD wastewater treatment and disposal facility consisting of a grit chamber, a 1000 gallon precast concrete oil/water seperator, a 1000 gallon precast concrete two compartment septic tank, a distribution box, 500 linear feet of three feet wide nitrification trench, piping, fittings and appurtenances to serve a truck washing pad at the Whiteville facility with no discharge to surface waters of the State. This permit is granted pursuant to the application received January 25, 1990 and revised February 1, 1990 in conformity with the project plans, specifications, and other supporting data, subsequently filed and approved by the Department of Environment, Health, and Natural Resources, and considered part of this permit. This permit shall be effective from the date of issuance until March 31, 1995 and shall be subject to the following specified conditions and limitations: 1. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. Adequate inspection, maintenance, and cleaning shall be provided by the permittee to insure proper operation of 'the subject facilities. 4. This permit is not transferable. 5. Upon the availability of a municipal or regional sewerage collection system, the subject wastewater treatment system shall be abandoned, and all wastewater discharged into the municipal or regional sewerage system. 6. This permit shall become voidable in the event of failure of the sail to adequately absorb the wastes, and may be rescinded unless the facilities are installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters and groundvaters. 7. The Wilmington Regional Office, Phone No. 919/256-4161 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed system so that an in-place inspection can be made of said system prior to backfilling. Such notification to the Regional Supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 8. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and groundwater protection will be established and an acceptable sampling reporting schedule shall be'followed. 9. The sludge generated from these treatment facilities must be disposed of in accordance with the General statutes 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 10. The wastewater treatment system shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor--- any --contamination of groundwaters which will render them unsatisfactory for normal use. Permit No. WQ0003030 Page -3- 11. Diversion or bypassing of the untreated wastewater from the wastewater treatment system is prohibited. 12.. The issuance of this permit shall not relieve the permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 13. No traffic or equipment shall be allowed on the disposal area except while installationoccurs or while normal maintenance is being performed. 14. An acceptable reserve area as indicated on the approved plans shall be maintained at all times. 15. The oil/water seperator shall be inspected weekly and pumped as necessary.
