A letter was sent to the permittee on November 4, 1994 by John Seymour of the
Permits & Engineering Unit, informing the permittee that his permit application was being
returned for failure to provide the Division with requested information.
The Permittee was then given until the original expiration date of the permit to provide
the needed information. The Division still has not received the requested information.
Therefore, Permit No. WQ0002685, which expired on December 31, 1994, will be
removed from our computer tracking systems, effective immediately.
By copy of this memo, l am requesting the Budget Office to clear all pending billings,
Notices of Revocation and permit file for the subject facility.
