BOX 278 AN(11ER, NC 27501 Dear Mr. Price: Subject: Permit No. WQ0002638 Town of Angier Wastewater Surface Irrigation Harnett County In accordance with additional information February 15, 2008 as required as part of the subject permit issued January 3, 2008 we are forwarding herewith a modified Attachment B, certified April 4, 2008, and updated location and soil maps to the Town of Angier for the continued operation of the subject wastewater treatment and surface irrigation facilities. This certification is being issued to modify Attachment B to: ♦ Update field locations and identification numbers. ♦ Verify field acreage. ♦ Verify dominant soil series classification for each field including drainage, and erosion classification. ♦ Establish annual loading rate for each field. This modified Attachment B shall be effective from the date of certification; shall void Attachment B of Permit No. WQ0002638 that was previously certified on January 3, 2008; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above - referenced permit. As always, remember to take the time to review this modified Attachment B thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No. WQ0002638, issued on January 3, 2008. Please replace the existing Attachment B of Permit No. WQ0002638 that was previously certified on January 3, 2008, with this modified Attachment B. Note that the vicinity, soils, monitoring well location, and acreage maps are attached to this modified Attachment B. It is the Perinittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment B and maps into Permit No. WQ0002638, issued on January 3, 2008, and snaking sure to retain any vicinity and buffer slaps for any existing and unmodified land application sites. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit, This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714.
