In accordance with a recommendation made by the Washington Regional Office on August 6, 2009, we
are forwarding herewith the corrected Attachment A of Permit No. WQ0002520, issued May 29, 2009, for
the subject facility. The following corrections have been made to the permit:
Attachment A — Limitations and Monitoring Requirements
1. Chloride (as Cl) - 00940, is being removed from the monitoring parameter for the PPI 001.
2. Solids, Total Dissolved -180 Deg. C - 70300, is being removed from the monitoring parameter for the
PPI 001.
3. 4 X year measurement frequency, is being changed to 3 X year.
Please replace Attachment A with the corrected Attachment A into Permit No. WQ0002520, issued
May 29, 2009.
