Dear Mr. Routh:
In accordance with your permit modification application package received on May 15, 2003, as well as
additional information received on May 23, 2003 and July 3, 2003, we are forwarding herewith a modified
Attachment B, certified on September 3, 2003, to the City of Reidsville for the subject residuals land application
program.
This certification is being issued to modify Attachment B. The modified Attachment B approves 388.2
acres of area for land application of residuals under this program.
Please replace the existing Attachment B of Permit No. WQ0002376 that was previously certified
on April 4, 2003, with this modified Attachment B. Note that the vicinity and buffers for any new and
modified land application sites are attached to this modified Attachment B. It is the Pennittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment B into Permit No.
WQ0002376, issued April 4, 2003, and making sure to retain any vicinity and buffer maps for any existing and
unmodified land application sites.
This modified Attachment B shall be effective from the date of certification; shall void Attachment B of
Permit No. WQ0002376 that was previously certified on April 4, 2003; and shall be subject to the conditions
and limitations as specified therein as well as those specified in the above referenced permit. As always,
remember to take the time to review this modified Attachment B thoroughly, as some of the conditions
contained therein may have been changed since the last certification. Note that this modified Attachment B is
hereby incorporated by reference into Permit No. WQ0002376, issued on April 4, 2003.
If any parts, requirements, and/or limitations contained in this certification or modified Attachment B
are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days
following receipt of the certification. This request shall be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative He at

This land application site is dominated by soils or site conditions that readily produce perched water table conditions or have very shallow depths to bedrock. Residuals
land application events shall be scheduled such that the total allowable PAN loading rate for a particular crop will not be applied in any single event. Events may occur
on this site throughout the year, provided that the vertical separation between the depth of residuals land application and the water table is greater than three feet.
e A 250-foot buffer shall be maintained around the water supply well that is located within the compliance boundary of this land application site. Within 30 calendar days
of this permit's issuance, but prior to initiating a residuals land application event on this site, the amended buffer map and revised net acreage value that takes this buffer
into account shall be submitted to the Division. Submit four copies of this information to: NCDENR-DWQ, NDPU Residuals Program Coordinator, 1617 Mail Service Center,
Raleigh, NC 27699-1617.
A reduced buffer from residences under separate ownership has been approved for this land application site. Residuals may be land applied to within 100 feet of the residences
owned by Robert Lee James (Mahe Marshall Road, Summerfield, NC 27359) and Samuel Kevin Cates (Summerfield, NC 27358), regardless of whether a surface
or a subsurface application method is used.
A reduced buffer from residences under separate ownership has been approved for this land application site. Residuals may be land applied to within 100 feet of the residences
owned by Tammy Davenport Bridges (8164 Mahe Marshall Road, Summerfield, NC 27358), regardless of whether a surface or a subsurface application method is used.
No land application of residuals shall take place on this land application site before a suitable vegetative cover has been established.
