In accordance with your application received on August 16, 1989, we are forwarding
herewith Permit No. WQ0002257, dated August 21, 1989, to Ambassador Baptist College for the
operation of the subject wastewater pump and haul activities.
This permit shall be effective from the date of issuance until December 31, 1989, and shall
be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are unacceptable to you,
you have the night to request an adjudicatory hearing upon written request within thirty (30) days
following receipt of this permit. This request must be in the form of a written petition, conforming
to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings, P.O. Drawer 11666, Raleigh, NC 27604. Unless such demands are made this permit
shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North
Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
Ambassador Baptist College
Cleveland County
FOR THE
operation of a 2,400 GPD wastewater pump and haul activity which will consist of pumping
wastewater out of a 3,000 gallon septic tank to be installed with high water alarm, pursuant to the
application received on August 16, 1989, and in conformity with the supporting data subsequently
filed and approved by the Department of Environment, Health, and Natural Resources and
considered a part of this permit.
This permit shall be effective from the date of issuance until December 31, 1989, and shall
be subject to the following specified conditions and limitations:
1. This pen -nit shall become voidable unless the facilities are operated in accordance with the
terms and conditions of this permit and the supporting information.
2. This permit is effective only with respect to the nature and volume of wastes described in
the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. The wastewater collected during this operation shall be adequately treated in the
Town of Boiling Springs Wastewater Treatment Facility prior to final disposal.
6. This permit shall become voidable unless the agreement between Ambassador Baptist
College and the Town of Boiling Springs for the treatment and disposal of the wastewater
is in full force and effect.
7. This permit shall become voidable unless the agreement between Ambassador Baptist
College and Wesson Septic Tank Service for the pumping and hauling of the wastewater is
in full force and effect.
8. This permit shall not be renewed or extended.
9. Ambassador Baptist College is liable for any damages which occur as a result of a spill
during this pump and haul operation.
10. No wastewater other than that from Ambassador Baptist College shall be included in this
pump and haul operation.
11. Adequate inspection, maintenance, and cleaning shall be provided by Ambassador Baptist
College to insure proper operation of these facilities.
12. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take such immediate corrective action as may be
required by this Division. This action may include the construction of additional or
replacement wastewater treatment or disposal facilities or the ceasing of the pump and haul
activities.
13. The holding tank shall be inspected as often as necessary in order to ensure that an
overflow does not occur.
14. The holding tank shall have an operating high water alarm which will be set at a level that
will provide sufficient storage to enable the hauler to l e summoned without danger of
overflow.
15. The Permittee shall maintain an accurate record of daily inspection and pumping activities
associated with this operation ( including frequency and amount pumped and hauled ).
This record shall be maintained by the Permittee for the life of the permit and shall be made
available to the Division of Environmental Management staff upon request.
