Heverly, Mayor Town of Emerald ISle 7500 Emerald Drive Emerald Isle, North Carolina 28557 Subject: Permit No. WQ0002249 The Town of Emerald Isle Carteret County Dear Ms. Heverly: In accordance with your permit application received August 14, 1989, we are forwarding herewith Permit No. WQ0002249 dated September 11, 1989 to the Town of Emerald Isle for the construction and operation of the subject wastewater treatment and disposal facility. This permit shall be effective from the date of issuance until September 30, 1994 and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to an adjudicatory heraring before a hearing officer upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Regional Supervi o EB:WQ2249.SEP cc: Carteret County HEalth Department Permits and Engineering Quible & Associates WiRO, CF NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION RALEIGH DEPARTMENT OF ENVIRONMENT, HEALTH AND NATURAL RESOURCES P E R M I T For the Discharge of Sewage, Industrial Wastes or other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations: PERMISSION IS'HEREBY GRANTED TO The Town of Ocean Isle for the construction and operation of a 2,500 GPD wastewater collection treatment and disposal facility consisting of a duplex 27 GPM pump station, 290 linear feet of 2 inch force main, 95 feet of & inch gravity sewer, a 3,000 gallon precast concrete two compartment septic tank, a 3,000 gallon duplex dosing tank, a 4200 square feet low pressure pipe disposal field, piping, fittings, and appurtenances to serve the Town of Ocean Isle Community Center, Public Works Building and Fire Station with no discharge to surface waters of the State. This permit is granted pursuant to the application received August 141 1989 in conformity with the project plans, specifications, and other supporting data, subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until September 30, 1994 and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. Permit No. WQ0002249 Page -2- 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. Adequate inspection, maintenance, and cleaning shall be provided by the permittee to insure proper operation of the subject facilities. 4. This permit is not transferable. S. Upon the availability of a municipal or regional sewerage collection system, the subject wastewater treatment system shall be abandoned, and all wastewater discharged into the municipal or regional sewerage system. 6. This permit shall become voidable in the event of failure of the sail to adequately absorb the wastes, and may be rescinded unless the facilities are installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters and groundwaters. 7. The Wilmington Regional Office, Phone No. 919/256-4161 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed system so that an in-place inspection can be made of said system prior to backfilling. Such notification to the Regional Supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 8. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and groundwater protection will be established and an acceptable sampling reporting schedule shall be followed. 9. The sludge generated from these treatment facilities must be disposed of in accordance with the General Statutes 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 10. The wastewater treatment system shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of groundwaters which will render them unsatisfactory for normal use. In event the facilities fail to perform satisfactorily, including the creation of nuisance conditions, or failure of the disposal area to adequately absorb the wastewater, the Permittee shall take such immediate corrective action as may be required by the Division of Environmental Management, including the construction of additional or replacement wastewater treatment or disposal facilities. Permit No. WQ0002249 Page -3- 11. Diversion or bypassing of the untreated wastewater from the wastewater treatment system is prohibited. 12. The issuance of this permit shall not relieve the permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 13. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 1.4. An acceptable reserve area as indicated on the approved plans shall be maintained at all times. 15. A suitable cover shall be maintained on the LPP field(s) and the field(s) shall be kept mowed. The clippings shall be removed to prevent the build up of thatch. 16. The low pressure lines shall be purged at least twice per year using potable water or air pressure adjusted on the distribution lines to approved specifications. t.
