Dear Mr. Vandiford:
In accordance with your application received July 6, 1989, we are forwarding
herewith Permit No. WQ0002003, dated August 31, 1989, to Mr. Carl Vandiford for the
construction and operation of the subject wastewater treatment disposal facility.
This permit shall be effective from the date of issuance until August 31, 1994, and
shall be subject to the conditions and limitations as specified therein. Issuance of this permit
hereby voids Permit No. 10763 issued September 28, 1984.
If any parts, requirements, or limitations contained in this permit are unacceptable to
you, you have the right to request an adjudicatory hearing upon written request within 30 days
following receipt of this permit. This request must be in the form of a written petition, conforming
to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative
Hearings, P.O. Box 11666, Raleigh, NC 27604. Unless such demands are made this permit shall
be final and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North
Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
operation of a 360 GPD single family residential wastewater treatment facility with final land
disposal of effluent via spray irrigation consisting of a 1,500 gallon septic tank, a subsurface sand
filter, a tablet type chlorinator, a 1,500 gallon storage tank, a 1,500 gallon pump tank with a 15
GPM submerged pump and valves, an on -off -alarm system, a 1.25 -inch force main, six (6) above
ground sprayers, a fenced spray field of 15,000 square feet (100 feet by 150 feet), piping, fittings,
and appurtenances to serve the Carl Vandiford, Jr., three (3) bedroom home located off NC 98,
pursuant to the application received July 6, 1989 and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the Department of
Environment, Health and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until August 31, 1994, and shall be
subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with the
approved plans, specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable until such time as the permittee has requested a name
change to a new permittee. Such a request must be accompanied by an application fee,
documentation from the new and current permittees, and other supporting data as may be
appropriate.
5. Upon the availability of a municipal or regional sewerage collection system, the subject
wastewater treatment facilities shall be abandoned and all wastewater discharged into the
municipal or regional sewerage system.
6. This permit shall become voidable in the event of failure of the soil to adequately absorb the
wastes, and may be rescinded unless the facilities are installed, maintained and operated in a
manner which will protect the assigned water quality standards of the surface waters and
ground waters.
7. The sludge generated from these treatment facilities must be disposed of in accordance with
General Statute 143-215.1 and in a manner approved by the North Carolina Division of
Environmental Management.
8. The spray irrigation facilities shall be effectively maintained and operated at all times so that
there is no discharge to the surface waters, nor any contamination of ground waters which
will render them unsatisfactory for normal use. In the event that the facilities fail to perform
satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to
adequately absorb the wastewater, the Permittee shall take such immediate corrective action
as may be required by the Division of Environmental Management.
9. A suitable vegetative cover shall be maintained.
10. Irrigation shall not be performed during inclement weather or when the ground is in a
condition that will cause runoff.
11. Adequate measures shall be taken to prevent wastewater runoff from the spray field.
12. Diversion or bypassing of the untreated wastewater from the treatment facilities is
prohibited.
13. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its
extension. Upon receipt of the request, the Commission will review the adequacy of the
facilities described therein, and if warranted, will extend the Permit for such period of time
and under such conditions and limitations as it may deem appropriate.
14. The application rates shall not exceed:
a. 0.048 inches/ 0.344 acres/day and 0.34 inches/ 0.344 acres/ week
b. the treated wastewater shall be uniformly applied so as to avoid runoff, ponding or
erosion
15. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to
surface or groundwaters resulting from the operation of this facility.
16. The facilities shall be effectively maintained and operated as a non -discharge system to
prevent the discharge of any wastewater resulting from the operation of this facility.
17. No type of wastewater other than that from subject three bedroom residence shall be sprayed
onto the irrigation area_
1 S. No traffic or equipment shall be allowed on the disposal area except while installation occurs
or while normal maintenance is being performed.
19. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure
proper operation of the subject facilities.
20. Prior to any transfer of this land, a notice shall be given to the new owner that gives full
details of the materials applied or incorporated at this site.
21. The Permittee is responsible for the operation and maintenance of the entire treatment and
disposal system including, but not limited to, the following items:
a. The septic tank shall be checked once per year and pumped out as needed
b. The tablet chlorinator shall be checked every week until the actual usage rate is
established and thence on a monthly basis. Tablets shall be added so as to provide
proper chlorination..
C. The storage system, pump system, and alarm system shall be
inspected monthly. The scum layer shall be removed at the same interval as the septic
tank is cleaned out.
d. The spray system shall be inspected monthly to make certain of
the sprayers proper operation, that the vegetative growth allows proper spray pattern,
that the soil is absorbing the disposed treated wastewater with no surface runoff, and
that no objectionable odors are being generated.
22. Diversion of bypassing of the untreated wastewater from the land application facilities is
prohibited.
23. The sand media of the subsurface sand filter must comply with the Division's specifications.
The engineer's certification will serve as proof of this compliance.
24. A set of approved plans and specifications for the subject project must be retained by the
applicant for the life of the project.
25. The septic tank, sand filter and irrigation field shall be located at least 100 feet away from any
drinking water well.
26. The spray irrigation field shall be fenced with at least a two strand wire fence.
27. Any monitoring deemed necessary by the Division of Environmental Management to insure
surface and ground water protection will be established and an acceptable sampling reporting
schedule shall be followed.
28. No sprayed wastewater shall be applied within 100 feet of the property boundary or any
water course.
29. Failure to abide by the conditions and limitations contained in this permit will subject the
Permittee to an enforcement action by the Division of Environmental Management in
accordance with North Carolina General Statutes 143-215.6.
30. Issuance of this permit hereby voids Permit No. 10763 issued September 28, 1984.
