Dear Mr. Scott:
Division of Water Resources' Central and Regional staff has reviewed your letter of July 5, 2017, requesting
fields SP04-06 and SP06-01 to -05 be added to the permit as a correction. However, in the permit renewal
package of 2015 the Land Application Site Certification (LASC) form submitted did not list these fields as
ones that were being renewed. Further, there were neither Land Owner Agreements nor maps provided
with the renewal package as would be required. For this reason, it was assumed that the fields were being
deleted and this matter cannot be treated as a correction.
Instead, the Town of Spindale should submit a request for permit modification to have these fields added
to the permit. Because the fields have been dropped, to add them back, the fields will need to be treated as
"new" fields during the modification process.
