Orr Regional Manager Amendment to Permit No.WQ0001921 Maintenance of Records Low Pressure Pipe System Cane River School Wastewater Treatment Facility Yancey County State Permit No. WQ0001921 was issued to Yancey County Schools for the construction and operation of a low pressure pipe (LPP) wastewater treatment system to serve the Cane River School. Condition No. 20 of that permit requires that adequate records be maintained of the system by the permittee. A review of the permit indicates that additional record keeping is necessary to indicate how the facility is operating; therefore, I am modifying Permit No_ WQ0001921 to include the following: 1. The Permittee or his designee shal_1_ inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors anc3 discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division of Environmental Management or other permitting authority. 2. A suitable cover shall be maintained on the LPP field(s) and the field(s) shall be kept mewed. The clippings shall be removed to prevent the build up of thatch. As an indicator of proper operation and maintenance, wastewater samples shall be taken from the septic tank influent, dosing chamber and sand filter effluent during October and April of each year and shall be analyzed for BOD5, TSS and pH. 4. Adequate records showing all maintenance shall be maintained by the Permittee. These records should include but are not limited to the following: Items Inspection Frequency a) Individual dosing pump operation (hours) Weekly b) Individual field rotation weekly c) High water alarms Weekly d) Residual pressure on highest and Monthly lowest lateral on each subfield 5. The low pressure lines shall be purged at the beginning and end of each school year using clean water and the pressure adjusted on the distribution lines to approved specifications. If any parts, requirements, or limitations contained in this permit modification are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the permit modification. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604.
