In accordance with your application recsived June 19, 1989, we
are forwarding herewith Permit Number WQ0001921, to the Yancey County
Board of Education, for the construction and operation of subject
wastewater treatment and disposal system to serve the Cane River Middle
School.
This permit shall be effective from the late of issuance until May
31, 1989, and shall be subject to the conditions and limitations as
specified therein.
If any parts, requirements, or limitations contained in this permit
are unacceptable to you, you have the right to request an adjudicatory
hearing upon written request within 30 days following receipt of this
permit. This request trust be in the form of a written petition,
conforming to Chapter 150B of North Carolina General Statutes, and
filed with the Office of Administrative Hearings, Post Office Drawer
11666, Raleigh, North Carolina 27604. Unless such demands are made
this permit shall be final and binding.

P E R M I T
For the Discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143,
General Statutes of North Carolina as amended, and other applicable
Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO

Cane River Middle School
FOR THE
construction and operation of a 4000 gpd low pressure pipe ground,
absorption wastewater treatment system consisting of an existing
septic tank followed by a subsurface sand falter, two -2000 gpd pump
tanks in series; each tank containing dual 185 gpm pumps, automatic
alternator, low water level indicator, second pump on and alarm float
controls and twenty-four hour timers to regulate dosing times and
quantities, a precast valve box, 4 -inch supply line, four low pressure
pipe disposal fields consisting of 54,000 square feet (10,800 lineal
feet), and all associated piping and appurtenances to serve the Cane
River Middle School with no discharge to the surface waters of the
State, pursuant to the application received June 19, 1989, and in
conformity with the project plans, specifications and other supporting
data subsequently filed with and approved by the Department of Natural.
Resources and Community Development and considered a part of this
permit.
This Permit shall be effective from the date of its issuance
until May 31, 1994, and shall be subject to the following specified
conditions and limitations:
1. The facilities shall be properly maintained and
operated at all times.
2. This Permit is effective only with respect to the
nature and volume of wastes described in the
application and other supporting data.
3. This Permit shall become voidable in event of failure
of the soil to adequately absorb the wastes, and may
be rescinded unless the facilities are installed,
maintained, and operated in a manner which will protect
the assigned water quality standards of the surface
and ground waters.

4. In the event the facilities fail to perform satis-
factorily, the permittee shall take such immediate
corrective action as may be required by this Division,
including the construction of additional or replacement
wastewater treatment or disposal facilities.
5. This Permit is not transferable.
6. The sludge generated from these treatment facilities
must be disposed of in accordance with General Statute
143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
7. This disposal system shall be abandoned
upon the availability of a regional sewer system.
9. Any monitoring deemed necessary by the Division of
Environmental Management to insure the protection of
ground waters will be established and an acceptable
sampling and reporting program shall be followed.
10. The approved low pressure pipe system must not be
disturbed, regraded, or substituted without written
permission from the Asheville Regional Supervisor.
11. This permit shall become voidable unless the facilities
are constructed in accordance with the approved plans,
specifications and other supporting data.
12. Upon completion of construction and prior to operation of
this permitted facility, a certification must be received
from a professional engineer certifying that the permitted
facility has been installed in accordance with the approved
plans and specifications.
13. The Asheville Regional Office, phone No. 704/251-6208, shall
be notified at least twenty-four (24) hours in advance of
backfilling of the installed low pressure pipe system so
that an in-place inspection can be made. Such notification
to the regional supervisor shall be made during normal
office hours from 8:00 a.m. until 5.00 p.m. on Monday
through Friday, excluding State holidays.
14. The Permittee, at least six (6) months prior to the
expiration of this Permit, shall request its extension.
Upon receipt of the request, the Commission will review the
adequacy of the facilities described therein, and if
warranted, will extend the Permit for such period of time
and under such conditions and limitations as it may deem
appropriate.

15. The Permittee shall employ a certified wastewater treatment
plant operator to be in responsible charge of the wastewater
treatment facilities. Such operator must hold a certificate
of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the
Certification Commission.
16. The facilities shall be effectively maintained and operated
as a non -discharge system to prevent the discharge of any
wastewater resulting from the operation of this facility.
17. Adequate inspection, maintenance and cleaning shall be
provided by the Permittee to insure proper operation of the
facilities.
18. An acceptable reserve area, equal in size to the approved
area, shall be maintained at all times.
19. A set of approved plans and specifications for the subject
project must be retained by the applicant for the life of
the project.
20. Adequate records shall be maintained by the Permittee.
These records should include but are not limited to the
following.

21. The disposal area will be maintained as a sports field with
a grass cover crop.
22. Prior to using the system, the permittee shall adjust
actual pressure head within each field to design
specifications.
