JAY ZIMMERMAN Director Subject: Attachment B Certification Permit No. WQ0001800 Town of Yadkinville RLAP Yadkin County In accordance with your permit modification request received July 11, 2017, and subsequent additional information received September 12, 2017, we are forwarding herewith a modified Attachment B certified September 25, 2017, to the Town of Yadkinville for the subject residuals program. This certification is being issued to modify Attachment B by: Addition of five fields in Yadkin County totaling 33.2 acres, said fields being NC-YK-27-01, NC-YK-28-01, NC-YK-28-02, NC-YK-28-03 and NC-YK-29-01. This modified Attachment B shall be effective from the date of certification until February 28, 2022 (i.e., expiration date of the current permit), shall void Attachment B of Permit No. WQ0001800 issued March 13, 2017, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace the Attachment B of Permit No. WQ0001800 that was issued March 13, 2017, with this modified Attachment B. Please note that only the maps for new land application sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, and/or limitations contained in this certification or modified Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
