WQ0001664, dated September 13, 2001, to Carolina Water Service, Inc. of NC for the construction and continued operation of the subject wastewater treatment reclaimed water utilization system. This permit has been modified to reflect the addition of two unlined disposal ponds. Please note Condition ILI regarding the disposal ponds. This permit has been limited to a maximum of 110,000 GPD because of the capacity of the proposed clarifiers. The wastewater treatment capacity shall remain at 110,000 GPD until the permit has been amended and plans have been submitted to add an additional clarifier. The existing Division of Environmental Health (DEH) subsurface system was not considered part of the review of the proposed spray irrigation system. Issuance of this permit does not authorize operation of the existing subsurface permit. If Carolina Water Service, Inc. of NC would like to maintain the existing subsurface system as a back-up to the proposed system, it will be the company's responsibility to maintain that subsurface disposal permit through DEH. This permit shall be effective from the date of issuance until August 31, 2006, shall void Permit No. WQ0001664 issued June 11, 2001, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This permit shall be effective from the date of issuance until August 31, 2006, shall void Permit No. WQ0001664 issued June 11, 2001, and shaII be subject to the following specified conditions and limitations: I. PERFORMANCE STANDARDS Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting materials. If this project is to be competed in phases and partially certified, you shall retain the resgonsibility to track further construction a roved under the same permit, and shall provide _a final certificate of completion once the entire project has been completed. Mail the Certification to the Non -Discharge Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617. 2. The Wilmington Regional Office, telephone number (910) 395-3900, shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5.00 p.m. on Monday through Friday, excluding State Holidays_ 3. The reclaimed water utilization facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. 4. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 5. The residuals generated from these treatment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the Division. 6. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 7. The following buffers shall be maintained: a) 100 feet between wetted areas and water supply wells, b) 100 feet between wetted areas and waters classified as SA, c) 25 feet between wetted areas and surface waters not classified as SA, d) 100 feet between wastewater treatment units and wells, e) 50 feet between reclaimed water storage/inigation ponds and property lines, and f) 50 feet between wastewater treatment units and property lines. Public access to the irrigation sites shall be controlled during active site use. Such controls may include the posting of signs showing the activities being conducted at each site. A sign shall be posted in plain sight in the club house showing these activities. 1I. OPERATION AND MAINTENANCE REQUIREMENTS The facilities shall be properly maintained and operated at all times. The disposal ponds on Fairways #2 and #4 shall not be used for irrigation purposes. Upon classification of the facility by the Water Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the WPCSOCC. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of 15A NCAC 8G .0202. The ORC of the facility must visit each Class I facility at least weekly and each Class IL III, and N facility at least daily, excluding weekends and holidays, and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of 15A NCAC 8G .0202. Once the facility is classified, the Permittee must submit a letter to the WPCSOCC which designates the operator in responsible charge within thirty days after the wastewater treatment facilities are 50% complete. A suitable, year-round vegetative cover shall be maintained on the irrigation areas. 4. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 5. Adequate measures shall be taken to prevent wastewater runoff from the spray field. 6. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 7. The application rate shall not exceed the following cumulative loadings over any twelve (12) month period: 96.7 inches per year on Fairways 1 *, 2, 4, 5, 6, 7, 15, 16, 17, 18 (*consisting of Baymeade, Autryville, and/or Pactolus soils) 32.2 inches per year on Fairways 1 *, 8, 9, 10, 11, 12, 13, and the driving range (*consisting of Foreston and/or Leon soils) The instantaneous application rate shall not exceed 0.4 inches per hour. 8. No type of wastewater other than that from Belvedere Plantation shall be sprayed onto the irrigation area. All wastewater shall be routed to the five-day holding pond should the limit for fecal coliform (daily maximum concentration of 25 per 100 mI) or turbidity (instantaneous maximum of 10 NTU) be exceeded, until such time that the problems associated with the treatment capability of the wastewater treatment plant have been corrected. The wastewater in the five-day holding pond shall be pumped back to the treatment plant for retreatment or treated in the five-day pond prior to discharge to the storage pond. M. MONITORING AND REPORTING REQUIREMENTS Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. Influent flow shall be continuously monitored and daily flow values shall be reported on Form NDMR. The Permittee shall install an appropriate flow measurement device consistent with approved engineering and scientific practices to ensure the accuracy and reliability of influent flow measurement. Flow measurement devices selected shall be capable of measuring flows with a maximum deviation of less than 10 percent from true flow, accurately calibrated at a minimum of once per year, and maintained to ensure that the accuracy of the measurements is consistent with the accepted capability of that type of device. Records of flow measurement device calibration shall be kept on file by the Permittee for a period of at least three years. At a minimum, data to be included in this documentation shall be. a. Date of flow measurement device calibration b. Name of person performing calibration c. Percent from true flow 3. During the remainder of the year, these parameters shall be monitored monthly. ** Triannually sampling shall be conducted during January, June, and October. If Groundwater sampling indicates or predicts problems with the compliance with Groundwater Standards, this permit will be modified to include additional and/or more restrictive Iimitations. 4. Adequate records shall be maintained by the Perm.ittee tracking the amount of wastewater disposed. These records shall include, but are not necessarily limited to the following information: a. date and time of irrigation, b, volume of wastewater irrigated, c. zone irrigated, d. length of time zone is irrigated, e. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each zone, f. weather conditions, and g. maintenance of cover crops. S. Three (3) copies of all operation and disposal records (as specified in condition III 2) on Form NDAR-I and three (3) copies of all effluent monitoring data (as specified in condition III 3 and III 4) on Form NDMR-1 shall be submitted on or before the last day of the following month. A record shaII be maintained of all residuals removed from this facility. This record shaII include the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept the residuals, date the residuals were hauled, and volume of residuals removed. 7. A maintenance log shall be maintained at this facility including but not limited to the following items: a. Daily sampling results including residual chlorine, settleable matter, and dissolved oxygen in the aeration basin and at the clarifier weir. b. Visual observations of the plant and plant site. c. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm testing, diffuser inspections and cleanings, etc.). d. Date of calibration of flow measurement device. e. Date and results of power interruption testing on alternate power supply. S. Noncompliance Notification: The Permittee shall report by telephone to the Wilmington Regional Office, telephone number (910) 395-3900, as soon as possible, but in no case more than 24 hours or on the nekt working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. IV. GROUNDWATER REQUIREMENTS Within 90 days of permit issuance, six monitor wells, shall be installed to monitor groundwater quality. The well(s) shall be constructed such that the water level in the well is never above or below the screened (open) portion of the well at any time during the year. The general location and name for each well is marked on Figure 3 and Figure 4. Each monitoring well shall be located at the review boundary, constructed in accordance with this permit, and approved by the Wilmington Regional Office. 2. All reports and any supporting documentation (GW-1, GW-30, GW-59) shall be sent to the following address: Groundwater Section Permits and Compliance Unit 1636 Mail Service Center Raleigh, NC, 27699-1636 Updated blank forms (GW-1, GW-30, GW-59) may be downloaded from the Groundwater Section's website at http://gw.ehnr.state.nc.us/ or requested from the address mentioned above. I Monitor wells MW-I, MW-2, MW-3, MW-4, MW-5, and MW-6 shall be sampled initially after construction and prior to waste disposal operations, and thereafter every January, June, and October for the parameters listed below. Prior to sampling the parameters, the measurement of water levels must be taken. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed relative to a common datum. Water Level pH Chlorides Total Dissolved Solids (TDS) Fecal Coliform Total Phosphorous Nitrate (NO3-N) Total Ammonia Nitrogen (NH3-N) The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality Monitoring: Compliance Report Form) by the Groundwater Section, Permits and Compliance Unit, 1636 Mail Service Center Raleigh, N.C. 27699-1636 on or before the last working day of the month following the sampling month. The data of all groundwater sampling analyses required by the permit conditions must be reported using the most recent GW-59 form along with attached copies of the laboratory analyses. Any laboratory selected to analyze parameters must be Division of Water Quality (DWQ) certified for those parameters required. 4. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. 5. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L, Groundwater Classifications and Standards. For disposal systems using reclaimed water regulation 15A 2H .0219(k) specifies that both the Compliance Boundary and the Review Boundary are established at the property boundary. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action according to 15A NCAC 2L .0106(d)(2). 6. The wells must be constructed by either a North Carolina Certified Well Contractor, the property owner, or the property lessee according to General Statutes 87-94.4(b)(2). If the construction is not performed by a certified well contractor, the property owner or lessee must physically perform the actual well construction activities, and the wells trust be constructed in accordance with 15A NCAC 2C .0108 (Standards of Construction for Wells Other than Water Supply) and any other state and local laws and regulations pertaining to well construction. 7. The Wilmington Regional Office, telephone number (910) 395-3900, shall be notified at least forty-eight (48) hours prior to the construction of any monitoring well so that an inspection can be made of the monitoring well location. Such notification to the regional groundwater supervisor shall be made during the normal office hours from 8:00 a.m until 5:00 p.m. on Monday through Friday, excluding state holidays. & Within sixty (60) days of completion of all monitoring wells, the permittee shall submit two original copies of a scaled topographic map (scale no greater than 1":100); however, special provisions may be granted upon prior approval for large properties. The reaps must be signed and sealed by a professional engineer or a state licensed land surveyor that indicates all of the following information: a. the location and identity of each monitoring well, b, the location of the waste disposal system, c. the location of all property boundaries within 500 feet of the disposal areas, d. the latitude and longitude of the established horizontal control monument, e. the relative elevation of the top of the well casing (which shall be known as the "measuring point"), and f. the depth of water below the measuring point at the time the measuring point is established., and g, the location of Review of Compliance boundaries. Control monuments shall be installed in such a manner and made of such materials that the monument will not be destroyed due to activities that may take place on the property. Maps and any supporting documentation shall be sent to the Groundwater Section as addressed in Condition IV.2, 9. Within thirty days of completion of all well construction activities, a certification must be received from a professional engineer or a licensed geologist certifying that the monitoring wells are located and constructed in accordance with the Well Construction Standards (15A NCAC 2C) and this permit. This certification should be submitted with copies of the Well Completion Form (GW-1) for each well. 10. For the initial sampling of the well as specified elsewhere in the permit, the permittee shall submit a copy of the GW-1 Form (Well Completion Form) with the Compliance Monitoring Form (GW- 59) for that well. Compliance Monitoring Forms that do not include copies of the GW-1 form will be returned to the permittee without being processed. Failure to submit these forms as required by this permit may result in the initiation of enforcement activities pursuant to NC General Statutes 143-215.6A. V. INSPECTIONS 1. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Pencnittee for a period of three years from the date of the inspection and shall be made available upon request to the Division or other permitting authority. Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. VI. GENERAL CONDITIONS This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request mast be submitted to the Division accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate_ The approval of this request will be considered on its merits and may or may not be approved. 4. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this pen -nit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 6. A set of approved plans and specifications for the subject project must be retained by the Permittee for the life of the project. 7. The annual administering and compliance fee must be paid by the Permittee within thirty days after being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15 NCAC 2H .0205 (c)(4). 8. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. Classification of Physical/Chemical Water Pollution Control Treatment Systems: Gracie I Physical/Chemical: Any water pollution control system that utilizes a primarily physical process to treat wastewater. This classification includes groundwater recnediation systems. ** Grade II Physical/Chemical: Any water pollution control system that utilizes a primani}f chemical process to treat wastewater. This classification includes reverse osmosis, electrodialysis, and ultrafthration systems. ** **.Any water pollution control system that wilizes a phyicaUchemical process to enhance an acris,nted sledge orfized growth process, shall not be subject to additional classification. Classification of Subsurface Water Pollution Control Systems: Systems which utilize the soil for subsurface treatment and disposal of wastewater and/or are recltrired to have a certified operator under 15A NCAC 18A.1961. *** Any subsurface s}astern that has as part of its treatment process a water pollution control sYsterrrs that may be classified under Rules .0302 through .0307 of this section shall he subject to addional classification. Definitions Activated Sludge - slialI mean a biological wastewater treatment process in which predominantly biodegradable polluntants in wastewater are absorbed, or adsorbed by living aerobic organisms and bacteria. Chemical Process - shall mean a wastewater pollution control system process consisting exclusively of the addition of chemicals to treat wastewater. Collection System - sliall mean a continuous connections of pipelines, conduits, pumping stations, and other related constructions used to conduct wastewater to the water pollution control system. Electrodialysis System - shall mean a system utlizing a selective separation of dissolved solids process that is based on electrical charge and diffusion through a semipermeable membrane. Physical Process - shall mean any water pollution control system process consisting of electrodialysis, adsorption, absorption, air stripping, gravimetric sedimentation, floation, and filtration as a means of treatment, Reverse Osmosis System - sliall mean a system which utilizes solutions and semipermable membranes to separate and treat wastewater. Ultrafiltration System - shall mean a system which utilizes a membrane filter to remove pollutants from wastewater. Water Pollution Control System - shall mean any system for the collection, treatment, or disposal of wastewater and is classified under the provision of G.S. 9OA-37.
