In accordance with your letter of February 2, 2009 authorizing the removal of Fields NC-UN-55-10 and
NC-UN-55-11 from the subject permit, we are forwarding herewith a modified Attachment B, certified March
9, 2009, to the City of Monroe for the subject residuals land application program.
This certification is being issued to modify Attachment B with the removal of 124.20 acres to already
permitted acreage, for a total of 3,036.94 acres in Union and Stanly Counties. Please replace the existing
Attachment B of the subject permit that was previously certified on June 5, 2008, with this modified
Attachment B. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this
modified Attachment B into Permit No. WQ0001346, issued on August 23, 2005.
This modified Attachment B shall be effective from the date of certification.; shall void the Attachment
B that was previously certified on June 5, 2008; and shall be subject to the conditions and limitations as
specified therein as well as those specified in the above -referenced permit. As always, remember to take the
time to review this modified Attachment B thoroughly, as some of the conditions contained therein may
have been changed since the last certification.
If any parts, requirements, and/or limitations contained in this certification or modified Attachment B
are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days
following the receipt of the certification.
This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh,
North Carolina 27699-6714. Unless such demands are made, this permit shall be final and binding.
