received July 31, 2015. However, additional information is required before the review may be
completed. This additional information letter supersedes that of August 3, 2015. Please address the
items on the attached pages no later than the close of business on September 14, 2015.

rules and regulations. Any oversights that occurred in the review of the subject application package
are still the Applicant's responsibility. In addition, any omissions made in responding to the
outstanding items in Sections A through D, or failure to provide the additional information on or
before the above requested date may result in your application being returned as incomplete. Items
listed under Section E — Recommendations are for your consideration, but do not require a response.

B. Residuals Manaeement Plan:
1. 15A NCAC 02T .1104 (b) states among other requirements that for modified residual
sources, location maps, TCLP analyses, quantitative justification, and sampling plans may
be required. The residual sources in the table below are new to the permit, yet these listed
requirements of 15A NCAC 02T. 1104 (b) have not been submitted. The first column of the
table is the permit number; the second is the facility name; the third is the number of dry
tons allowed in the 2014 Attachment A recertification; the fourth is the number of tons
proposed; the fifth is the number of tons the source provided in 2014 (2015 annual report);
the sixth is the TCLP requirement in the 2014 Attachment A recertification; and the seventh
is the county where the facility is located.
B. Operations and Maintenance Plan:
A copy of the operations and maintenance plan must be submitted with every
renewal package according to the instructions on RLAP 03-15. Please make sure
that all of the parts required in the operations and maintenance plan are included.
C. Land Owner Aereements:
No Land Owner Agreements (LDAs) were submitted for the Nash County Fisher fields,
8015-01-1 to 3. Please submit LOAs or indicate you wish to delete these fields from the
application in the Land Application Site Certification submitted.
2. No LOAs were submitted for fields JM -1 to 8. Please submit LOAs or indicate you wish to
delete these fields from the application in the Land Application Site Certification submitted.
3. The fields designated 4801 -WP -1 to 4 have an LOA signed by Jeanne Watson Cooper with
no contact information given. The Franklin County tax map (id 1854-55-3341) and deed
information (deed book / page : 996 / 923) show the land registered to Patricia W. Keith and
others of P. O. Box 516, Franklinton, NC 27525. Please provide verification that Jeanne
Watson Cooper is able to sign for Patricia W. Keith and others.
4. The fields designated JTB-02 to -07 have an LOA signed by Kathryn Bryant of Nebraska
Road, Enfield, NC. The Halifax County tax map (id 385800065511) and deed information

III can sign for his father.
D. Operations and Maintenance Plan
1. No operations and maintenance plan was provided with the renewal package as required in
the instructions. Please submit a complete operations and maintenance plan.
E. Recommendations: (Response not required)
1. Because there are parcels in multiple counties, I would suggest that the parcels be renamed
to include a prefix indicating the county in which the land is located. For instance, land in
Franklin County would be given a prefix of NC -FR- followed by the existing field name.
The 8015-01 and 8015-02 designation could then be simply 8015. The following suggested
prefixes would be used:
