JAY ZIMMERMAN Director Subject: Attachment A Certification Permit No. WQ0000838 GFI RLAP Granville County In accordance with your permit modification request received April 17, 2017, we are forwarding herewith a modified Attachment A certified May 11, 2017, to Granville Farms, Inc. for the subject residuals program. This certification is being issued to modify Attachment A by: 1. Addition of five new sources totaling 910 dry tons per year, those sources being (1) Mallinckrodt WWTP (CHEM, City of Raleigh Pretreatment Permit, 790 dry tons/year); (2) The Preserve at Jordan Lake (WQ0018146, 20 dry tons/year); (3) The Legacy at Jordan Lake (WQ0024844, 20 dry tons/year); (4) Briar Chapel Development (WQ0028552, 30 dry tons/year); and Hasentree Golf Community (WQ0033816, 50 dry tons/year). 2. Change in frequency for TCLP sampling to "Once per permit cycle" for the following facilities eight facilities: (1) SGWASA WTP (02-39-107); (2) Enfield WTP (04-42-025); (3) Spring Hope WWTP (NC0020061); (4) Tar River Water Reclamation Facility (NC0020231); (5) Henderson WRF (NC0020559); (6) Warrenton WWTP (NC0020834); (7) Kenly Regional WWTP (NC0064891); and (8) Franklin County WWTP (NC0069311). This modified Attachment A shall be effective from the date of certification until September 30, 2021 (i.e., expiration date of the current permit), shall void Attachment A of Permit No. WQ000083 8 issued October 11, 2016, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace the Attachment A of Permit No. WQ0000838 that was issued October 11, 2016 with this modified Attachment A. Please note that the residuals sources in Attachment A are listed in alphabetical order by permit number. If any parts, requirements, and/or limitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit shall be final and binding. Class A water treatment plant residuals are exempt from meeting the pathogen & vector attraction reduction requirements in Condition IVA, provided they are not mixed with residuals that have been generated during the treatment of domestic wastewater, the treatment of animal processing wastewater, or the biological treatment of industrial wastewater. The Class A water treatment plant residuals must be treated and processed in a manner as to not meet the definition of `Biological Residuals" as defined in I5A NCAC 021' .1102(6). If no land application events occur during a required sampling period (e.g. no land application occur during an entire year when annual monitoring is required), then no sampling data is required during the period of inactivity. The annual report shall include an explanation for missing sampling data. Those required to submit the annual report to EPA may be required to make up the missed sampling, contact the EPA for additional information and clarification.
