The Division is in receipt of information which confirms that the subject facility has been
completely dismantled, the flume removed, and the settling ponds backfilled. In addition, no
correspondence has been answered in reference to this facility, Le, billing for the annual
administrating and compliance monitoring fee. Therefore, this is to hereby advise you that the
Division is hereby rescinding the subject permit effective sixty days from the date of this notice. If
it is not your desire to have this permit rescinded, you must provide written notice to the Division
within this sixty day notice period.
