Dear Ms. Miller:
In accordance with your application received October 5, 1.988, we are
forwarding herewith Permit No. WQ0000580, dated October 14, 1988, to Ms. Anita. M.
Miller for the construction and operation of the subject pump and haul wastewater
disposal operations.
This permit shall be effective from the date of issuance until November 15,
1988, and shall. be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must:
be in the form of a written, petition, conforming to Chapter 150E of North. Carolina
General Statutes, and filed with the Office of Administrative hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604, Unless such demands are made this
permit shall be final. and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter, 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
Ms. Anita M. Miller

FOR THE
construction and operation of a 3000 GPD pump and haul disposal system to transport
wastewater from the Ross Plantation Restaurant (formerly Windmill Restaurant) to the
Lauradale Subdivision wastewater treatment plant, pursuant to the application
received October 5, 1988, and in conformity with the supporting data subsequently
filed and approved by the Department of Natural. Resources and Community Development
and considered a part of this permit.
This permit shall be effective from the date of issuance until November 15, 1988,
and shall be subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. The sewage and wastewater collected by this system shall be treated in the
Lauradale Subdivision Wastewater Treatment Plant prior to being discharged into
the receiving stream.
S. This permit is not transferable.
6. This permit shall become voidable unless the agreement between Ms. Anita Miller
and Scientific Water and Sewerage, Inc. for the collection. and final treatment of
wastewater is in full force and effect. .
7. The Permittee is liable for any damages caused by a spill or failure of the pump
and haul operations.

i 8. The issuance of this permit; shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
9. No wastewater other than that from Ross Plantation Restaurant shall be included
in this pump and haul operation.
1.0. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
11. This permit shall not be renewed or extended.

In accordance with a letter dated October 3, 1988, we are amending Permit No.
15820 to relocate the six (6) sprinkler sprCty irrigation field and associated valves,
piping and appurtenances as proposed in the supporting information, This amendment
will. also change the following:
1. Permit No. 15820 is changed to WQ0000579.
2. Condition No. 19 is amended as follows:
Prior to beginning waste disposal operations, four (4) monitor wells, one
(1) upgradient of the spray field, two (2) downgradient of the spray
field, and one (1) downgradien.t of the lagoon must be installed to
monitor groundwater quality. The location and construction details fo)_
these wells must be approved by the Mooresville Regional Office, from
which a well construction permit is ;egtired. The well locations approved
by the Mooresville Regional orrice may differ from the locations indicated
on the approved plans.
This permit amendment does not alter any other conditions or limitations
