Note that only the vicinity maps for any new and modified residuals source -generating facilities are attached to this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A and vicinity maps into Permit No. WQ0000455, issued on June 18, 2004, and making sure to retain any vicinity maps for any existing and unmodified residuals source -generating facilities. This modified Attachment A shall be effective from the date of certification; shall void Attachment A of Permit No. WQ0000455 that was previously certified on August 15, 2006; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above - referenced permit. As always, remember to take the time to review this modified Attachment A thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0000455, issued on June 18, 2004. If any parts, requirements, and/or limitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the from of a written petition, conforming to Chapter 150E of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. 'Residuals from this residual source -generating facility shall be incorporated into the sail within six hours after land application to meet vector attraction reduction, as the percent solids in these residuals are not sufficient to meet vector attraction reduction requirements through air drying alone.
