4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The project is an improvement of an existing roadway facility, Sugar Hill Road, over I-40. Ebsting conditions in the
project area include the roadway and bridge, adjacent forested areas along streams, and primarily business land
use. The bridge and culverts were built in 1958.

4f. Explain the purpose of the proposed project:*
The purpose of this project is to widen SR 1001 (Sugar Hill Road) to a three lane road. Pedestrian accessibility will
also be provided along SR 1001. In addition, improving the horizontal curve radii for the I-40 EB on-off ramps and
providing proper acceleration lane length for the I-40 EB and WB on-ramps will improve safety.
4g. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
This section of SR 1001 has two lanes e�sting. The proposed roadway will have three 12' lanes with curb & gutter
and sidewalk on the east side of the road. The proposed typical section also includes curb & gutter on both sides
with sidewalk. The ewsting bridge over I-40 will be replaced as well. In addition, the horizontal curve radii for the EB
on-off ramps will be flattened, the EB acceleration lane onto I-40 will be lengthened and the WB acceleration lane
onto I-40 will be lengthened. The roadway will be designed with a 50 mph design speed. Traffic will be maintained
on-site during construction except for tie-ins for the ramps and SR 1190 (Worley Road). Standard construction
equipment will be used to construct the project.

1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:*
Due to the topography and presence of multiple streams, avoidance of impacts was not feasible for this project.
However, impacts have been minimized to the greatest extent practicable and consist of three culvert extensions, a
pipe e�ension, and associated rip-rap bank stabilization. Side slopes have been designed as steep as possible
wfiile retaining stability in order to minimize impacts. Rip-rap bank stabilization has been designed to reduce outlet
velocities and prevent downstream erosion. The inlet extension at Site 2 has been adjusted to improve stream flow
alignment over ebsting conditions.

Sediment and erosion control measures will be implemented during construction and inspected on a regular basis.
Ditches have been lined with rip-rap to reduce velocity of stormwater entering jurisdictional waters. Culvert
extensions will be constructed in the dry using NCDOT Best Management Practices.
