Ms. Alexander,
Attached, please find the NPDES Rescission Request form for the former Alevo Manufacturing facility in Concord, North
Carolina (General Permit No. NCG030000 and Issuance of COC No. NCG030682). Alevo filed for bankruptcy earlier in the
year and all manufacturing operations have ceased and manufacturing equipment has been removed from the facility.
We will also provide a hard copy of the attached Rescission Request form via USPS. If you have any questions, please let
us know.
