Attach a vicinity map (i.e. street map) clearly showing the location of the property with respect to local landmarks such as towns, rivers, and roads. 2. Provide a detailed site plan showing property boundaries and proposed locations of vegetation clearing, structures (buildings, retaining walls, docks, impervious surfaces, etc.), rip rap, excavation or dredging below Full Pond/ Normal Water Level elevations, FORM: SSGP 10-2013 Page 1 of 3 and construction access corridors. You may use the diagram provided at the end of this application form. 3. Attach a photograph of the shoreline/ buffer proposed to be stabilized. (Include a scale of some sort- a yard stick, shovel handle, etc.) 4. Norton Rd. up the NW side of Lake Glenville. About 3/4 the way up the lake, there is a sign on your right to the entrance of Strawberry Hills subdivision and Strawberry Hill Road. Drive down Strawberry Hill Rd until you get to 926 Strawberry Hill Rd (Lt 10) on your right. Lt 9 is between 926 Strawberry Hill Rd (Lt 10) and 900 Strawberry Hill Road. You can park down the drive of the residence at 926 Strawberry Hill Road. Describe proposed work (include discussion as to how hardening of shoreline has been avoided, or why it is necessary): Lot 9: There is severe bank erosion killing (felling) trees and causing property erosion, etc. As lot 9 has been undeveloped and previous owner lived out of state, this has been an issue for many years. Current owner of lot 9 also owns adiacent lot 10 (PIN # 7554-68-0492). Owner would like to repair and clean up existing stone seawall on Lot 10 and continue stone seawall across Lot 10 (where incomplete) and across Lot 9 to stabilize shoreline and plan to develop Lot 9 for personal use. 9. Total amount of disturbance (including all excavation, backfill, rip rap, retaining walls, etc.) below the normal pool lake level/ Normal Water Level in square feet or acres: The length of shoreline for both properties combined is 308.40 lineal feet. Lot 10 already has an existing seawall (about 94 lineal feet) along most of its shoreline (about total 155 lineal feet). This seawall will be cleaned up and new natural stone (not rip rap) to support base to prevent erosion and the wall extended where not completed to tie into Lot 9 boundary and FORM: SSGP 10-2013 Page 2 of 4 extended across Lot 9 property shoreline. The wall will be stacked local stone and average about 4-5 feet in height and about 1-2 feet in width. Natural stone (not rip rap) similar to the wall will support wall about 6 feet out at base of wall to prevent erosion. 11. Total amount of disturbance (including all clearing, back fill, excavation, rip rap, retaining walls, etc.) above the normal pool lake level/ Normal Water Level and 50 feet land -ward in square feet or acres: None 12. Please describe the vegetation above the normal pool lake level/ Normal Water Level and 50 feet landward to be impacted (number of trees, for instance): None, we will bring stone down to bank via driveway and lot. Do not plan on cutting any trees down in 50 ft. setback or lot.
