The project shall be constructed in accordance with your modified application da[ed received September 20, 2018.
All the authorized ac[ivities and condi[ions associa[ed with the ori�inal Water Quality Certification da[ed July 31,
2018 s[ill apply eacept where superseded by this certifica[ion. After reviewing your application, we have decided
that these impac[s are covered by General Water Quality Certification Number 4132 and 4133. These certitications
correspond [o the Na[ionwide Penni[s 3 and 12 respectively issued by the Corps of Engineers. In addition, you
should acquire any o[her federal, state or local pemiits before you proceed w ith your project includine (but not
limited to) Sediment and Erosion Control, Non-Discharge and Water Supply Watershed re�uiations. This approval
will expire wi[h the accompanying 404 perniit.
This approval is valid solely for the purpose and design described in your application (unless modified below).

�vith alI the conditions. If total wetland fills for this prqject (now ur in the future) exceed one acre, or of total
impacts to streams (now or in the fu[ure) exceed 300 linear teet, compensa[ory mi[ication may he required as
described in I SA NCAC 2H .0506 Qi) (6) and (7). Additional buffer impacts may require compensarory mitigation
as described in I SA NCAC 26.0?59. For this approval to remain valid, you must adhere to the conditions listed in
the attached certifica[ion(s) and any addi[ional condi[ions listed below.
CondiYion(s) of Certification:
This moditication is applicable only to the additional proposed activities. All of the authorized activities and
conditions of certification associated with the orieinal Water Quality Certification dated July ; I. Z018 still apply
except where superseded by this certification.
2. A copy of this Water Quality Certitication shall be maintained on the co�struction site at all times. ln addition, the
Water Qualiry Cenitication and all subsequent moditications, ifany, shall be maintained with the Division Eneineer
and the on-site project manager. [ISA NCAC 0?hl .0507(c) and I SA NCAC 02H .0506 (b)(2) and (c)(2)]
