After completing a thorough investigation and a review of our Standard Operating Procedures, we
suspect the cause for the fecal exceedance that occurred on 6/19/2018 was due to lab error or
contaminated sampling containers. We have replaced all sampling containers to minimize this risk going
forward. Unfortunately, we failed to collect the weekly ammonia sample from the week of July 16,
2018. We sincerely apologize for this oversight which was partially caused by assigning anew ORC to
this facility. We have taken steps to insure this does not happen again.
