Please be advised that T A Loving Company takes this NOD very seriously.
The following is documentation of our efforts to address and correct the deficiency on our B-5604 Perquimans County
project (NOD) and to review all DOT projects to confirm compliance with the NPDES requirements.
Upon receipt of the NOD notification, I forwarded the NOD notification to our direct supervisors assigned to this
project — I requested them to review this NOD immediately and to discuss this NOD with the Resident Engineer's
staff. The following T A Loving personnel are in receipt of my email of August 14, 2018:
a. Stephen Salter—General Superintendent, Assistant Vice President
b. Chris Myers — Project Manager
c. Terry Jackson — Superintendent
d. Garland Roundtree -Foremen
2. On the morning of Wednesday August 15, 2018, I attended our monthly meeting for the C204037 Chowan
County project. I took this opportunity to review the NOD with Michael Hill, his staff and our project staff in an
effort to insure the NPDES forms for this project were in compliance.
On the afternoon of Wednesday August 15, 2018, Stephen Salter, Chris Myers and myself inet with David Otts,
his staff and Randy Midgett (This meeting was previously scheduled to discuss the recently awarded DA00333
Dare County project). At the conclusion of the DA00333 Dare County project meeting, I requested an
opportunity to discuss this NOD issued on the B-5604 Perquimans County project. We notified David Otts, his
staff and Randy Midgett that T A Loving Company accepted full responsibility of the NOD and informed them we
would meet on site with Terry Jackson at the conclusion of this meeting and review, not only what happened,
but also the proper documentation requirements of the NPDES forms.
4. On Monday August 20, 2018, I forwarded an email containing attachments for "NPDES Inspection Refresher
Information" received from the Department to assist our staff with properly completing the NPDES forms on all
NCDOT projects. The following T A Loving personnel are in receipt of my email of August 20, 2018:
a. Stephen Salter—General Superintendent, Assistant Vice President
b. Chris Myers — Project Manager
c. Clay Wisniewski — Project Manager
d. Justin Hill, PE — Project Manager
e. Terry Jackson — Superintendent
f. Matt Bierstedt - Superintendent
g. AK Reeves — Superintendent
h. BarryReeves—Superintendent
i. Jamie Allen — Superintendent
5. I have personally discussed this NOD with all of the above listed T A Loving personnel in an effort to notify them
of the Department's "No Tolerance" policy and T A Loving Company's expectations.
We will monitor our Superintendent's efforts to properly complete the NPDES forms. We will pro-actively discuss the
documentation of the SDO's with DOT field personnel on all current and future NCDOT projects.
