B-5604 Bridge 19 Replacement Project on US 17 Business

This letter is in response to the NC Division of Water Resources Notice of
Deficiency (NOD-2018-PC-0261) issued on August 14, 2019 due to Yncomplete
NCGO1000 inspection forms on the B-5604 Bridge 16 Project in Perquimans County.
NCDOT and the contractor, T.A. Loving Company, regret this incident and
corrected the deficiency immediately. We have met with the contractor and NCDOT
personnel involved with the project. Attached is a response from Mark Perkins with T.A.
Loving Company that provides a detailed explanation of the corrective actions taken and
of efforts to insure future compliance.
Additionally, within Division 1, we have reiterated these requirements and
expectations with all of our personnel involved with project inspections and compliance
with NCDWR permits. Refresher training is also being provided.
Please know that we take these actions very seriously and every effort will be
made to insure that such deficiencies do not occur again.
