Dear Ms. Driver:
A review has been conducted of the Sanitary Sewer Overflow (SSO) 5-Day Report(s)
submitted by the City of Winston-Salem for the month of July 2018. This review has shown
the City of Winston-Salem to be in violation of the requirements found in the WQCS00003
collection system permit and/or North Carolina General Statute 143.215.1(a)(1) The
violations are summarized below:

actions for SSOs on volume spilled, volume reaching surface waters, duration, gravity,
impacts to public health, fish kills, and other factors. DWR may pursue enforcement action
for these and/or any additional violations of the WQCS00003 collection system permit. If
violations are of a continuing nature, not related to operation and/or maintenance issues,
and you anticipate remedial construction activities, then you may wish to consider
applying for a Special Order by Consent (SOC).
