We have been working with Army Corps agent Ross Sullivan on this project. We conducted a confirmation visit on-site
and he agreed with our jurisdictional determinations. He wants to complete the JD request and NW permit together, so
we do not have written documentation yet. Same for the Nationwide Permit #, we confirmed over the phone but do not
have written documentation.
Should I have him email you to confirm both of the above?
Also, part D, section 6 is a data entry error.

I started reviewing the above referenced project and the following is required in order to process the application:
• Please provide documentation for review that the U.S. Army Corps of Engineers has verified all stream, wetland,
and/or open water locations for the project. [15A NCAC 02H .0502(a)(7)]
• Please provide documentation for review that the U.S. Army Corps of Engineers has verified the Nationwide
Permitthat is to be used to authorize this project. [15A NCAC 02H .0502(a)(7)]
• In Part D, section 6: Buffer Impacts Area 2 and 3.1 state these impacts are allowable with mitigation. However,
in Part E, Section 2a. the application states the project does not require compensatory mitigation. Please
confirm part D, section 6 is a data entry error.

I process the PCN's in the order that I receive them. Currently, from the date they are received in Central Office, I am 5-6
weeks out from when they are submitted. Hope this gives you an idea when I will begin the review process.

Your work is authorized by the above referenced permit provided itis accomplished in strict accordance with the enclosed
Conditions, your application signed and dated 7/25/2018, and the enclosed plans (2017 Sewer Systems Improvements signed and
sealed 7/23/2018 and the Stream & Wetland Impact Maps dated July 2018). Any violation of the attached conditions or deviation
fromyour submitted plans may subject the permittee to a stop work order, a restoration order, a Class I administrative penalty,
and/or appropriate legal action.
This verification will remain valid until the expiration date identified below unless the nationwide authorization is modified, suspended
or revoked. If, prior to the expiration date identified below, the nationwide permit authorization is reissued and/or modified, this
verification will remain valid until the expiration date identified below, provided it complies with all requirements of the modified
nationwide permit. If the nationwide permit authorization expires or is suspended, revoked, or ismodified, such that the activity would
no longer comply with the terms and conditions of the nationwide permit, activities which have commenced (i.e., are under construction)
or are under contract to commence in reliance upon the nationwide permit, will remain authorized provided the activity is completed
within twelve months of the date of the nationwide permit's expiration, modification or revocation, unless discretionary authority has
been exercised on a case-by-case basis to modify, suspend or revoke the authorization.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification, You
should contact the NC Division of Water Resources (telephone 919-807-6300) to determine Section 401 requirements.
