Comment: The records reviewed durina the inspection were oraanized and well maintained. Discharae
Monitoring Reports (eDMRs) were reviewed for the period November 2017 through June
2018. Daily maximum effluent fecal coliform and TSS violations were reported in March
2018 (fecal coliform) and April 2018 (TSS). The Division has separately addressed these
limit violations through the issuances of Notice of Violations (NOVs).
No effluent total nitrogen and total phosphorus sampling was performed during the first
quarter of 2018. This inspection report will address these monitoring violations.
No indication of the ORC and/or Backup ORC being onsite was reported on the May 2018
edmr for the week of 5/20/18 and 5/29/18 — 5/30/18. A revised May 2018 edmr should be
resubmitted to include this information.

Envirolink's field laboratory certification #5212. Statesville Analytical, Inc. (all effluent
parameters except field) has also been contracted to provide analytical support.
The ORC was not performing post -analysis calibration checks on the pH and total residual
chlorine meters following field analyses at multiple sites. Post -analysis calibration checks
must be performed in accordance with the NC Wastewater/Groundwater laboratory
certification approved procedures for the analyses of dissolved oxygen, PH, and total
residual chlorine. Note: The technical guidance documents developed by the Division's
Laboratory Certification Unit detailing these approved laboratory procedures have been
emailed to Envirolink's Field SUDervisor.

with a clear supernate. The aeration basin dissolved oxygen levels were low (<1.0 mg/L);
however, the wastewater appeared to be adequately treated. The ORC must ensure
adequate dissolved oxygen levels are maintained in the aeration basin at all times.
The Soda ash is added on an as -needed basis to maintain appropriate alkalinity/PH levels.
