The Division of Water Quality (DWQ) has determined that your application was incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. Therefore, unless we receive the additional information requested below, we will have to move toward denial of your application as required by 15A NCAC 211.0506 and will place this project on hold as incomplete until we receive this additional information. Please provide the following information so that we may continue to review your project. Additional Information Requested: The DWQ requests that the wetland flags be reflagged around the areas where the road crosses through wetlands and areas of adjacent wetland impacts. The DWQ conducted a site visit on Monday June 22, 2009 and found areas that looked like wetlands had been impacted with sediment. The DWQ met with David Murphy on site (6/22/09) and told him about the area. The DWQ came back on Thursday June 25, 2009 to reinspect the removal of sediment within the wetland area. It was very difficult to determine if the impacts had been removed due to the missing wetland flags and the existing road alignment compared to the proposed road alignment. For wetland Impacts # 3b, 4, 5, and 6 no pipes are shown on the plan view or cross sections. To provide for interconnectivity of hydrology between wetlands the DWQ would require pipes. Please include pipes and modify the permit application accordingly. On sheet C-1 in Section lI around impacts # 5, the upland islands look as if there are lot lines on them. There are no proposed impacts showing road crossings to access these areas. Please address this issue and modify permit application accordingly.
