The North Carolina Division of Water Resources conducted an inspection of the Point Emerald Villas WWTF on
August 13, 2018. This inspection was conducted to verify that the facility is operating in compliance with the
conditions and limitations specified in Non -discharge Permit No. WQ0000224. A summary of the findings and
comments noted during the inspection are provided in the enclosed copy of the inspection report.
The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Wilmington
Regional Office. The following deficiency(s) was noted during a file review and site inspection:
Inspection Area Description of Deficiency
Monitoring Wells Monitoring well cap (MW -5) was open during inspection. Well cap and lock were corroded.
This deficiency was noted in previous inspection report (07/26/2016).
End Use -Infiltration Deteriorating conditions of the rotary distributor.

Inspection Area Description of Deficiency
Record Keeping Monitoring well sampling and analysis results for active wells (MW -1, MW -4, MW -5) should be
submitted to the address below. This deficiency was noted in previous inspection report
(07/26/2016).

Remedial actions should be taken to correct these deficiencies problem and prevent further occurrences in the
future. The Division of Water Resources may pursue enforcement action for this and any additional violations of
State law. To prevent further action, carefully review these deficiencies and address the causes of
non-compliance to prevent the recurrence of similar situations. A written response is expected within 30 days of
receiving this notice. The response should include a plan of action for each deficiency and a time table for
completion.
