Ext.-Ph.II Location: NC 122 North & Various County: Edgecombe River Basin: Tar-Pamlico DWQ Project Number: 09-0567 Dear Ms. Karoly: Please find five (5) copies of the attached modified engineering drawings. The plan has been modified to address the comments from your letter dated May 22, 2009. Our responses to your comments are as follows: 1. Please denote stream impact numbers on the site plans and enumerate impacts to buffers and streams on the plans. Additionally, please verify stream impact length on page 6 of the PCN application. Stream and buffer impacts have been enumerated on the plans. Stream impact lengths have been updated from 173 linear feet to 123 linear feet since we have decided to directionally drill under an additional stream. We have also adjusted a buffer that was incorrectly shown running along the centerline of Cherry Hill Church Rd. to follow an intermittent stream just north of the road. This adjustment eliminates the buffer impacts along this route on sheet 6. Updated PCN Form pages with buffer and stream impacts are attached. 301 West 14th Street Greenville NC 27834 2. Please provide typical details of waterline stream crossings that are referenced in the submitted plans. 252.757.1096 tax 252.757.3221 Waterline stream crossing details for both wet and dry conditions have been added to the drawings on sheet 21. If you have any questions, please do not hesitate to contact our office. Please note that this is a Rural Center funded project that must be under construction by August 20, 2009. An expeditious review of this permit would be appreciated. Best regarJ /'J ' 4EilOSUres: . H E. The perennial and one intermittent streams are being directionally drilled, therefore, no impacts are expected. Buffer impact identification labels have been added to the plan set. D. Impact Justification and Mitigation 1. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project. Lines will be constructed in existing roadside ditch and in existing right-of-way, which is all previously-disturbed land. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques. Minimizing trench width, minimizing sediment runoff by using sedimentation and erosion control measures, directionally-drilling under all perennial streams and immediate seeding and mulching. 2. Compensatory Mitigation for Impacts to Waters of the U.S or Waters of the State . 2a. Does the project require Compensatory Mitigation for ? Yes ® No impacts to Waters of the U.S. or Waters of the State? 2b. If yes, mitigation is required by (check all that apply): ? DWQ ? Corps ? Mitigation bank 2c. Will the project result in an impact within a protected riparian buffer that requires buffer mitigation? ? Yes ® No 6b. If buffer mitigation is required, discuss what type of mitigation is proposed (e.g., payment to private mitigation bank, permittee responsible riparian buffer restoration, payment into an approved in-lieu fee fund). The riparian buffers that will be impacted will be restored to pre-construction quality and appearance. Minimal vegetation will be removed in the buffer zones during stream crossings. All Zone 2 disturbance will be replanted with appropriate seed mixture and mulched. All Zone 1 disturbance will be replaced with ditch liner of existing material type. 6h. Comments: Impacts in wetted area of open cut streams will be restored with erosion control matting to previous state and other impacts in each dry zone will be regraded and reseeded to previous appearance and aualitv.
