Inspection Summary:
Inspection with Dan Fortin and staff. Permit expired 9/30/16. Permittee has submitted renewal application twice, but the
app has been returned twice due to failure to provide required additional information, most recently 5/1/18. Current issues
pending include primarily multiple ownership of site parcels and lack of recorded unrestricted access easements, lease
agreements etc required by 02T.0704(f). The facilties remain in noncomliance for this issue. In addition, the following was
noted:
1) The 'old' treatment trains 1 and 2 are currently used for emergency storage as indicated by the operators. Both tanks
have visible significant corrosion, and it recommended that the tanks not be used until a structural engineer can inspect and
make recommendations for usage. The end of tank 2 is buckled outward with several top tank welds broken loose. It is
recommended this tank be pumped out and removed from service immediately;
2) The active trains are missing walkway gratings creating a safety hazard for operators. Gratings should be installed in
these areas and corroded gratings in other locations replaced as determined to be needed by operators;
3) The primary flow equalization tank has excessive grease that needs to be pumped out. The grease can foul pump and
alarm control floats and create excessive odors;
4) Drip irrigation field no. 2 is in need of grass cutting so that routine inspection of drip lines can be made;
5) Monitoring well data (GW -59 forms) have not been received for the following months: 6/16, 6/17, 10/17, 2/18. If theses
analyses have been performed, please submit the data on the appropriate forms. Lack of submittal of this data is a
monitoring violation in accordance with permit WQ0011313 and may result in further enforcement action

Comment: Facilties were permitted for nutrient removal via an anoxic system; construction was started but
not completed. Currently, the plant is being operated as extended aeration plant (2 trains in
operation rated at 82,000 gpd). One anoxic basin was constructed and is being used for
additional flow equalization. The original trains 1 and 2 are being used for'upset' storage and
additional EQ during emergencies. The tanks are badly corroded and are recommended to be
taken off-line unless an independent structural analysis is performed. The side wall at the end of
tank no. 2 is buckled outward and this tank should be taken off-line immediately

Comment: UV unit on train 3 is inoperable, operators use chlorine tablets
placed in clarifier troughs and the

Comment: Property including drip zones is currently subdivided with 4 different owners. Prior to permit
renewal (expired 9/30/16) , unrestricted permittee access easments will need to be approved
and recorded. Permittee, Peppertree Owner's Association, is currently in bankruptcy precedings
according to the site manager.
