Inspection Summary:
An inspection of the Town of Oak Island was conducted on 08-16-2018. The facility was compliant with Permit W00005790.
A reminder that, according to Attachment A of the permit, weekly monitoring of effluent starts from May 1 st through August
31st. Staff indicated that May's reports would be resubmitted with the correct monitoring frequency. Also, the reporting of
PPI 002 and 003 was reviewed with staff. Customized forms will be made and emailed to help minimize reporting confusion.

Comment: SCADA is set up to call the on-call operator first for any alarms. If alarm is not acknowledged, it
will call other operators then the wastewater superintendent. During inspection, high water audio
and visual alarms were activated when float was pulled, but Scada did not show alarm. Staff
stated that it would be checked and/or repaired as soon as possible.

record calibrations, field parameter test results, process control test results, pond levels and
lime addition. The facility is field parameter certified, Certification #5692. In July, 2017, the Total
Nitrate Nitrogen limit for the effluent was removed from the permit. There is a limit of 10mg/I Total
Nitrate Nitrogen for the groundwater monitoring wells. GW59 reports indicate violations of Nitrate
limits for MW #8 from 2012 to 2017. The Regional Office will continue to watch these trends.
