Dear Mr. Raper:
On June 9, 2009,the Division of Water Quality(DWQ)received your application dated June 3, 2009,to
fill or otherwise impact 0.31 acres of 404/wetland and 250 square feet of Zone 1 Tar-Pamlico River basin
protected riparian buffers and 4,250 square feet of Zone 2 Tar-Pamlico River basin protected riparian
buffers to extend an 18"PVC sanitary sewer and install 29 manholes at the site. The DWQ has
determined that your application was incomplete and/or provided inaccurate information as discussed
below. The DWQ will require additional information in order to process your application to impact
protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the
additional information requested below,we will place this project on hold as incomplete until we receive
this additional information. If we do not receive the requested information,your project will be formally
returned as incomplete. Please provide the following information so that we may continue to review your
project.
Additional Information Requested:
1. Please locate the project boundaries on the most recent Nash County bound and published county
soil survey.
2. Please provide a detailed planting plan for the buffer replanting(standard format includes:
scientific name, common name, source of the plant material,plant spacing, wetland indicator
status, etc., as well as planting specifications).
3. Please provide stream determination documentation on the site(upstream and downstream photos
and DWQ stream identification forms). At least 2 features, one near MH No. 83, and one near
MH No. 72, and possibly MH No. 43, do not appear to be included on the plans.
4. Can the wetland impacts at Site No. 1 be avoided by moving the line between the two wetlands?

6. Please provide documentation demonstrating why the wetland impacts at Site No. 3 cannot be
avoided by moving the line approximately 100 feet north?
7. Please provide documentation demonstrating why the line cannot be moved out of buffer Zone 2
at Buffer Impact Site No. 1?
8. Please provide documentation demonstrating why the line cannot be moved to avoid impacts at
Buffer Impact Site No. 2?
9. Are you proposing a maintenance corridor? If so,please provide the impacts for the maintenance
corridor.
Please submit this information within 30 calendar days of the date of this letter. If we do not receive this
requested information within 30 calendar days of the date of this letter,your project will be withdrawn and
you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands,waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at
